Premalignant Skin Lesions among Residents of a Rural Municipality of Rio Grande do Sul, Brazil

Resumo: | Introduction: Skin cancer is an important public health issue and, despite the relevance of the topic, there are few studies available in Brazil addressing the pre-malignant skin lesions. Objectives: To describe the characteristics of rural population that works in an outdoor environment and analyse the association with the presence of premalignant skin lesions. Method: A population survey was conducted in a rural county in the South of the country between the years of 2010-2011. 242 individuals participated in this study. The subjects reported working outdoors, were in the age group of ≥40 years old and both genders were included. Interviews were conducted, and dermatologists of the regional referral hospital network evaluated the presence of skin lesions. All pre-malignant skin lesions in the study population were defined as outcome. Results: The prevalence of skin lesions in the study population was of 29.3% (71 cases). In a rough association analysis was observed that individuals aged ≥60 years were four times more likely to have skin lesions than those who were younger (OR=4.05; IC95% 1.89-8.70). The skin type I- II had a triple chance to have skin lesions compared to other skin types (OR=2.99; IC95% 1.58- 5.66) and those working in agriculture were twice as likely to have skin lesions in relation to other activities (OR=2.04; IC95% 1.01-4.14). Conclusion: Individuals with skin photo type I-II and workers performing their duties outdoors are part of specific groups with a higher risk for skin cancer. Strategies for the prevention and early detection of skin lesions should be implemented in these priority groups. |

Introducción: El cáncer de piel es un problema de salud pública muy importante, sin embargo, hay pocos estudios disponibles en Brasil que hablan de las lesiones cutáneas premalignas. Objetivos: Describir las características de trabajadores que ejercen sus actividades laborales al aire libre y analizar la asociación con la presencia de lesiones cutáneas premalignas. Método: Encuesta de la población se llevó a cabo en una ciudad rural en el sur del país, entre 2010-2011. En este estudio participaron 242 personas de edades ≥40 anos, de ambos sexos, que informaron trabajar al aire libre. Se realizaron entrevistas y dermatológos de la red de hospitales de referencia regional evaluaron la presencia de lesiones en la piel. Todas las lesiones cutáneas pré-malignas en la población de estudio se definieron como resultado. Resultados: La prevalencia de lesiones de la piel en la población estudiada fue de 29,3% (71 casos). En el análisis de asociación se observó que las personas de edad ≥60 años eran cuatro veces más propensos a tener lesiones en la piel que los que eran más jovenes (OR=4,05; IC95% 1,89-8,70). También fue observada una mayor probabilidad de desarrollar lesiones cutáneas entre los participantes clasificados como fototipo de piel I- II con relación a los de outros tipo de piel (OR=2,99; IC95% 1,58- 5,66) y los que trabajaban en la agricultura o pecuaria con relación a los que laboraban en otras actividades (OR=2,04; IC95% 1,01-4,14). Conclusión: Las personas con fototipo de piel I- II y los trabajadores que realizan sus tareas al aire libre forman parte de grupos específicos con mayor riesgo de cáncer de piel. Estrategias para la prevención y la detección precoz de lesiones en la piel deben ser implementadas en estos grupos.Introdução: O câncer de pele e um problema de saúde pública relevante; mas; apesar disso, há poucos estudos disponibilizados no Brasil que discorrem sobre as lesões pré-malignas da pele. Objetivos: Descrever as características de uma população rural, que exerce suas atividades laborais ao ar livre, e analisar a associação quanto a presença de lesões cutâneas pré-malignas. Método: Inquérito populacional realizado em um município rural do Sul do país, entre 2010-2011. Participaram desse estudo 242 indivíduos com idade ≥40 anos, de ambos os sexos, que relataram trabalhar ao ar livre. Entrevistas foram realizadas e médicos dermatologistas da Rede de Referência Hospitalar Regional avaliaram presença de lesões na pele. Foram definidas como desfecho todas as lesões cutâneas pré-malignas encontradas nessa população. Resultados: A prevalência de lesões cutâneas na população de estudo foi de 29,3% (71 casos). Na analise de associação ajustada, observou-se que a chance dos indivíduos com idade ≥60 anos terem lesões cutâneas foi quatro vezes maior em relação aos que tinham menor idade (OR=4,05; IC95% 1,89-8,70). Uma maior chance de desenvolver lesões cutâneas também foi observada entre os participantes classificados como fototipo de pele I -II em relação aos de outros tipos de pele (OR=2,99; IC95% 1,58- 5,66), e os que trabalhavam na agricultura e/ou pecuária em relação as outras atividades (OR=2,04; IC95% 1,01-4,14). Conclusão: Indivíduos com fototipo de pele I- II e trabalhadores que exercem suas funções ao ar livre fazem parte de grupos específicos com maior risco para cancer de pele. Estratégias de prevenção e detecção precoce das lesões cutâneas devem ser prioritariamente implementadas nesses grupos.
