Cognitive Function in Patients Undergoing Chemotherapy: a Systematic Review

Resumo: Introduction: Studies suggest that, in addition to the systemic effects, chemotherapy can lead to cognitive alterations in patients. These changes have had increasing attention due to the repercussion that these effects cause in patients who survive a cancer treatment. Objective: To study the domains of cognitive function impaired in patients who underwent chemotherapy, by means of a systematic review of the evaluation methods used to identify the presence of these alterations and their possible mechanisms of occurrence. Method: A systematic review was conducted from a bibliographic survey of articles from the last 10 years, in the Lilacs, Medline and Pubmed database. the key words were randomly combined in Portuguese: “quimioterapia”, “cognicao”, “neurotoxicidade”, “antineoplasicos”, “cancer”, and in English "chemobrain", "Neurocognitive function", "chemofog." Results: The cognitive function presented itself impaired after chemotherapy was carried out. A large array of evaluation methods of the cognitive function was found, highlighting the lack of standardization. The possible mechanisms for which chemotheraphy provokes impairment are related to the capacity of crossing the blood-brain barrier; DNA damage, regulation of cytokines, neural repair, genetic alteration of neurotransmitters, and changes in the levels of estrogen and testosterone hormones. Conclusion: The following cognitive domains: verbal memory, working memory, executive function, attention, concentration, language and motor speed are the most affected in patients undergoing chemotherapy.

Estas alteraciones reciben creciente atención debido a la repercusión que eses efectos causan en la vida de aquellos que supervivieran a un tratamiento para el cáncer. Objetivo: Estudiar la función cognitiva em pacientes sometidos la quimioterapia. Método: Se realizó una revisión integradora desde una revision bibliográfica de artículos de los últimos 10 años, la base de datos Lilacs, Medline y PubMed. Se combinan de forma aleatoria las palabras clave en portugués "quimioterapia", "cognición", "neurotoxicidad", "antineoplásico", "cáncer", e Inglés "chemobrain", "neurocognitive function", "chemofog". Resultados: La función cognitiva se presentó perjudicada después de la realización de la quimioterapia en diversos ámbitos. Una diversidad de métodos de evaluación de la función cognitiva fue encontrada evidenciando una falla de patrón. Los posibles mecanismos por los cuales la quimioterapia favorece el perjuicio cognitivo están relacionados a la capacidad de atravesar la barrera hematoencefálica, daños al DNA, regulación de las citocinas, reparación neural, alteración genética de neurotransmisores y alteración en los niveles de hormonas estrógeno y testosterona. Conclusión: Los dominios cognitivos memoria verbal, memoria de trabajo, función ejecutiva, atención, concentración, lenguaje y velocidad motora son los más perjudicados en pacientes sometidos a la quimioterapia.Introdução: Estudos sugerem que, além dos efeitos sistêmicos, a quimioterapia pode levar a alterações cognitivas nos pacientes. Essas alterações tem recebido crescente atenção devido à repercussão que esses efeitos causam na vida daqueles que sobreviveram após um tratamento para o câncer. Objetivo: Estudar os domínios da função cognitiva prejudicados em pacientes submetidos a quimioterapia, através de uma revisão integrativa sobre os métodos de avaliação utilizados para determinar a presença dessas alterações e os seus possíveis mecanismos de ocorrência. Método: Realizou-se uma revisão integrativa a partir de levantamento bibliográfico de artigos dos últimos dez anos, na base de dados Lilacs, Medline e Pubmed. Foram combinadas aleatoriamente as palavras-chave em português: “quimioterapia”, “cognição”, “neurotoxicidade”, “antineoplásicos”, “câncer”; e em inglês: “chemobrain”, “neurocognitive function”,”chemofog”. Resultados: A função cognitiva apresenta-se prejudicada após a realização da quimioterapia em diversos domínios. Uma vasta gama de métodos de avaliação da função cognitiva foi encontrada, evidenciando uma ausência de padronização. Os possíveis mecanismos pelos quais a quimioterapia provoca prejuízo cognitivo estão relacionados a capacidade de atravessar a barreira hematocefálica; danos ao DNA; regulação das citocinas; reparo neural; alteração genética de neurotransmissores; e alteração nos níveis de hormônios estrógeno e testosterona.
