Mastology Assistance in a Reference Unit of the Unified Health System in Ceara, Brazil

Resumo: Introduction: The time of breast cancer diagnosis has direct influence on the evolution and prognosis of the disease, and the precocity of the diagnosis is directly proportional to the chances of cure. Therefore, it is important to identify limitations for early diagnosis. Objective: To evaluate the care provided to patients in a reference unit in Mastology in Ceara, according to their origin. Method: Cross-sectional and quantitative study. A structured questionnaire was administered to users of outpatients of the Mastology Institute of Cancer Prevention in Ceara who had been under care during the year 2010. Results: Among the 600 women interviewed, 139 (23.2%) were diagnosed with breast cancer, 50.4% of whom lived in the capital and 49.6% in the countryside. The mean waiting time for a specialist consultation was 15 days for women living in the State capital and 30 days for those coming from the countryside, being twice as high (p=0.0001). When the surgical procedure was indicated, the average waiting time for residents in the capital was 39 days, and for those coming from the countryside, 63 days (p=0.046). The time to perform the biopsy was adequate for more than 90% of women, but this did not occur when it came to performing surgery. Among the women, 491 (81.8%) were satisfied with the service. Conclusion: The delay in performing surgery may cause a negative impact in the patient´s survival and reduce the chances of cure, especially for those who dwell in the hinterland of the State.

Por lo tanto, es importante identificar las limitaciones para el diagnóstico precoz. Objetivo: Evaluar la atención prestada a los pacientes de una unidad de referencia Mastología en Ceará, en conformidad con su procedencia. Método: Estudio transversal y cuantitativo. Fue utilizado un cuestionario estructurado que se aplicó a las usuarias del ambulatorio de mastología, del Instituto de Prevención del Cáncer de Ceará, atendidas durante el año 2010. Resultados: Entre las 600 mujeres entrevistadas, 139 (23,2%) tenían diagnóstico del cáncer de mama, el 50,4% de ellas viven en la capital y 49,6% provienen del interior. El tiempo promedio de espera para una consulta especializada fue de 15 días para las mujeres que viven en la capital y 30 días para las provenientes del interior, es el doble de tiempo (p=0,0001). Cuando el procedimiento quirúrgico se indicó, el tiempo promedio de espera fue de 39 días para capital y 63 días para el interior (p=0,046). El tiempo para realizar la biopsia fue adecuado para más de 90% de las mujeres, pero el mismo no ha ocurrido a la hora de realizar la cirugía. Entre las mujeres, 491 (81,8%) se mostraron satisfechas con el servicio. Conclusión: El retraso en la realización de la cirugía puede influir negativamente en la supervivencia de los pacientes y reducir las posibilidades de curación, sobre todo para aquellas residentes en interior del Estado.Introdução: O momento do diagnóstico do câncer de mama influencia diretamente na evolução e prognóstico da doença, sendo a precocidade desse diagnóstico diretamente proporcional as chances de cura. Objetivo: Avaliar a assistência prestada às pacientes em uma unidade de referência em mastologia no Ceará, de acordo com sua procedencia. Método: Estudo transversal e quantitativo. Foi aplicado um questionário estruturado para usuárias do ambulatório de mastologia do Instituto de Prevenção do Câncer do Ceará, atendidas durante o ano de 2010. Resultados: Entre as 600 mulheres entrevistadas, 139 (23,2%) tinham diagnóstico de câncer de mama, sendo 50,4% residentes na capital e 49,6% procedentes do interior. A mediana de tempo de espera por uma consulta especializada foi de 15 dias para mulheres residentes na capital e 30 dias para aquelas vindas do interior, sendo duas vezes maior (p=0,0001). Quando o procedimento cirúrgico foi indicado, a média do tempo de espera para residentes na capital foi de 39 dias e, para as procedentes do interior, 63 dias (p=0,046). O tempo para realização das biópsias foi adequado para mais de 90% das mulheres, porém o mesmo não ocorreu quando foi necessário realização de cirurgia. Entre as mulheres, 491 (81,8%) estavam satisfeitas com o atendimento.
