Comfort Diet in Oncologic Palliative Care: Reflections on Food-related Meanings of Comfort

Resumo: | Food plays an important role in people’s lives, not only for physiological reasons, but also considering social and psychological aspects involving food. Due to the global epidemiological transition, a large portion of the population lives with several chronic diseases, among them cancer. In this context, palliative care emerges as an approach that aims to improve the quality of life of patients and their families, through the prevention and relief of suffering, early identification, and treatment of pain and other physical, psychosocial, and spiritual problems. In advanced disease, changes in eating experiences often bring great suffering, and the “comfort diet” is widely used in this clinical practice, but its definition has no consensus yet. This article seeks to evoke reflections on what “comfort diet” involves in the area of oncologic palliative care. Comfort can be defined as a multidimensional phenomenon, an experience strengthened if the needs related to relief, tranquility and transcendence are met in four contexts: physical, psychospiritual, sociocultural and environmental. The diet may be able to provide comfort in all these aspects, by relieving symptoms, rescuing affective memories, strengthening the bond with family members, and maximizing life potential in face of the illness. Individualization must guide the decision making about the diet, considering the several dimensions of comfort and understanding all the subjectivity that this word can assume. |

Debido a la transición epidemiológica mundial, una gran parte de la población vive con una serie de enfermedades crónicas, entre ellas el cáncer. En este contexto, los cuidados paliativos surgen como un enfoque que pretende mejorar la calidad de vida de pacientes y familiares, mediante la prevención y el alivio del sufrimiento, la identificación precoz, el tratamiento del dolor y otros problemas de naturaleza física, psicosocial y espiritual. En la enfermedad avanzada, los cambios en las experiencias alimentarias a menudo conllevan un gran sufrimiento, y la “dieta de confort” es ampliamente utilizada en esta práctica clínica, aunque sin un consenso sobre su definición. Este artículo pretende evocar reflexiones sobre lo que engloba la “dieta de confort” en el ámbito de los cuidados paliativos oncológicos. El confort puede ser definido como un fenómeno multidimensional, siendo una experiencia fortalecida por la satisfacción de necesidades de alivio, tranquilidad y trascendencia, atendidas en cuatro contextos: físico, psicoespiritual, sociocultural y ambiental. La dieta puede ser capaz de reconfortar en todos estos aspectos, a través del alivio de los síntomas, rescatando recuerdos afectivos, fortaleciendo el vínculo con los familiares y maximizando el potencial de la vida frente a la enfermedad. La individualización debe ser la guía para la toma de decisiones sobre la dieta, considerando las diversas dimensiones del confort y comprendiendo toda la subjetividad que un término puede asumir.A alimentação possui um papel importante na vida das pessoas, não apenas por razões fisiológicas, mas também considerando aspectos sociais e psicológicos que envolvem a comida. Em razão da transição epidemiológica global, grande parcela da população convive com uma série de doenças crônicas, entre elas o câncer. Nesse contexto, os cuidados paliativos emergem como uma abordagem que visa à melhoria da qualidade de vida de pacientes e familiares, por meio da prevenção e alívio do sofrimento, da identificação precoce, do tratamento da dor e de outros problemas de natureza física, psicossocial e espiritual. Na doença avançada, frequentemente alterações nas experiências alimentares trazem grande sofrimento, sendo a “dieta de conforto” muito utilizada nessa prática clínica, porém sem um consenso sobre sua definição. Este artigo busca evocar reflexões sobre o que engloba a “dieta de conforto” na área dos cuidados paliativos oncológicos. O conforto pode ser definido como um fenômeno multidimensional, sendo uma experiência fortalecida pela satisfação das necessidades de alívio, tranquilidade e transcendência, atendidas em quatro contextos: físico, psicoespiritual, sociocultural e ambiental.
