Breast Cancer Detection: Knowledge, Attitude and Practices of Doctors and Nurses from the Family Health Strategy of Mossoró, RN, Brazil

Resumo: Introduction: Breast cancer is a heterogeneous and complex disease observed by multiple forms of clinical and morphological presentations, degrees of tumor aggressiveness and metastasis potential. In this way, in order to comply with the Unified Health System principles, the Ministry of Health prioritized the family as the basic unit for assistance. Objective: To investigate the knowledge, attitude and practices of doctors and nurses from the Family Health Care Strategy in Mossoro, Rio Grande do Norte, Brazil, as to breast cancer early detection. Methodology: A cross-sectional and inferential study. Data was collected in March 2008 and a self-applied questionnaire was given to 33 doctors and 47 nurses whose average age was 42. Results: It was observed that the years at work and the high turnover rate among health care professionals made it difficult for their capacitation. There is a valorization of the clinical breast exam in detriment to a mammography request to early diagnose breast cancer. An insufficient number of skilled health professionals and the population’s lack of knowledge are limiting factors to screen breast cancer. Young women in the high risk group do not usually receive a mammography request. Conclusion: It was observed a need to qualify these professionals to consolidate the National Program for Permanent Health Education, increase the offer of mammograms and promote health education for the general public.

En este sentido para cumplir con los principios del Sistema de Salud, el Ministerio de Salud ha dado prioridad a la familia como elemento básico para la asistencia. Objetivo: Investigar el conocimiento, actitud y prácticas de los médicos y enfermeros en la Estratégia de la Salud de la Família en Mossoró (RN) con respecto a la detección temprana del cáncer de mama. Métodos: Estudio descriptivo, transversal e inferencial, en que la recogida de datos tuvo lugar en marzo de 2008, fue utilizado cuestionario auto aplicable. Participaron 33 médicos y 47 enfermeros, con edad media de 42 años. Resultados: El tiempo de trabajo y el volumen de actuación en la atención básica entre los profesionales de la salud em el servicio, dificulta su formación. Hay una valorización del examen clínico de la mama, en lugar de la solicitud de la mamografía en busca del diagnóstico precoz del cáncer de mama. La escasez de profesionales de salud cualificados y el desconocimiento de la población son factores limitantes para el rastreamiento del cáncer de mama. Hay poca práctica de se hacer la mamografía en las mujeres jóvenes en alto riesgo. Conclusión: Hay una necesidad de profesionales cualificados para fortalecer el Programa Nacional de Educación Continua en Salud, aumentar la oferta de exámenes de mamografía y de promover la educación para la salud de la población.  Introdução: O câncer de mama é uma doença heterogênea e complexa, observada pelas múltiplas formas de apresentação clínica e morfológica, pelos graus de agressividade tumoral e potencial metastático. Neste sentido, para atender aos princípios do Sistema Único de Saúde, o Ministério da Saúde priorizou a família como elemento básico para a assistência. Objetivo: Investigar o conhecimento, atitude e práticas dos médicos e enfermeiros da Estratégia Saúde da Família de Mossoró (RN) com relação à detecção precoce do câncer de mama. Metodologia: Estudo transversal e inferencial. A coleta dos dados realizou-se em março de 2008 com a utilização de questionário autoaplicável. Participaram 33 médicos e 47 enfermeiros com idade média de 42 anos. Resultados: O tempo de atuação na atenção básica e a grande rotatividade entre os profissionais de saúde no serviço dificultam sua capacitação. Há uma valorização do exame clínico das mamas, em detrimento à solicitação da mamografia na busca do diagnóstico precoce do câncer de mama. A insuficiência de profissionais de saúde habilitados e o desconhecimento da população são fatores limitadores ao rastreamento do câncer de mama. Ocorre solicitação reduzida da mamografia em mulheres jovens de alto risco.
