"Adjuvant Surgery" in the Treatment of Metastatic Gastrointestinal Stromal Tumor's: a New Concept

Resumo: Recent progress in gastrointestinal stromal tumor's (GIST) treatment was responsible for changing GIST's natural history. Knowlegde acquirement on molecular mechanism-based systemic therapy gave rise to the development of targeted antineoplastic drugs capable of reaching outcomes that had never been reached before. The introduction of imatinib in the clinical practice not only changed GIST patients´ survival, but also shifted paradigms. However, although imatinib has been considered the standard treatment of metastatic GIST, surgeon's role in this setting is being investigated. There is some evidence that surgery after imatinib treatment would benefit selected patients with metastatic disease. Therefore, a new concept has developed: "adjuvant surgery" as a new strategy to optimize treatment outcome in GIST metastatic scenario.

El mejor conocimiento de los mecanismos moleculares ha sido fundamental para el desarrollo de la terapia molecular dirigida, capaz de obtener resultados hasta entonces jamás logrados en ningún tratamiento de tumor sólido. La introducción del imatinib en la práctica clínica ha cambiado no sólo la supervivencia de los pacientes con GIST metastásico, sino también fue responsable de la quiebra de paradigmas bien establecidos, pero, a pesar del tratamiento de la enfermedad metastásica estar basada en la utilización del imatinib, el papel del cirujano, en este contexto, ha sido revisado, habiendo algunos indicios de que la cirugía de rescate en la enfermedad metastásica puede traer beneficios a grupos seleccionados de pacientes. Por lo tanto, un nuevo concepto se ha establecido, el de la "cirugía adyuvante", es decir, una estrategia terapéutica que apunta a optimizar los resultados del tratamiento del GIST metastásico.Os recentes avanços no tratamento dos tumores estromais gastrointestinais (GIST) foram responsáveis pela mudança da história natural da doença. O melhor conhecimento dos mecanismos moleculares foi fundamental para o desenvolvimento de droga alvo-molecular dirigida capaz de obter resultados, até então, não alcançados em nenhum tratamento de tumor sólido. A introdução do imatinibe na prática clínica modificou não só a sobrevida dos portadores de GIST metastático, como também foi responsável por quebras de paradigmas bem estabelecidos; porém, apesar de o tratamento da doença metastática ser baseado na utilização do imatinibe, o papel do cirurgião, nesse contexto, tem sido revisado, havendo alguns indícios de que a cirurgia de resgate na doença metastática possa trazer benefícios a grupos selecionados de pacientes.
