Quality of Life and Shoulder Motion after Surgery for Breast Cancer: Physical Therapy Focus

Resumo: Introduction: Complications after surgery for breast cancer can cause injury in range of motion of the shoulder and a negative impact considering the quality of life. Objective: To evaluate the range of motion and quality of life before and after ten sessions of physiotherapy in postoperative of breast cancer. Method: A clinical self-controlled trial was conducted involving 36 women. The range of motion was assessed by goniometry of homolateral and contralateral (control) shoulder. Quality of life was evaluated by the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C-30 (EORTC QLQ-C30) and Breast Cancer Module (BR-23). The dependent and independent Student t test was used with p<0.05. Results: After the 10th sessionwe found a significant improvement in flexion (p<0.001), extension (p<0.001), abduction (p<0.001), adduction (p <0.010), medial rotation (p<0.011) and lateral rotation (p<0.020). When compared to the control member, such movements were similar except abduction that, although close to functional values, was still lower than the control limb (p<0.029). There was a significant improvement of physical function (p=0.004), pain reduction (p=0.046), financial difficulties (p=0.008) and symptoms in the arm (p=0.019). Conclusion: The physiotherapeutical approach improved the range of motion and quality of life in women after surgery for breast cancer. It is suggested that accompaniments for longer time may bring additional benefits.

Objetivo: Comparar la amplitud del movimiento y la calidad de vida antes y después de diez sesiones de fisioterapia en el postoperatorio del cáncer de mama. Método: Se ha realizado un ensayo clínico autocontrolado con 36 mujeres. La amplitud del movimiento fue evaluada por la goniometria del hombro homolateral y contralateral (control). La calidad de vida fue evaluada por la encuesta European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C-30 (EORTC QLQ-C30) y Breast Cancer Module (BR-23). Se ha utilizado la prueba t de Student dependiente e independiente, adoptándose p<0,05. Resultados: Después de la décima sesión, encontraron una mejoría significativa en la flexion (p<0,001), extension (p<0,001), abducción (p<0,001), aducción (p<0,010), rotación medial (p<0,011) y la rotación lateral (p<0,020). Cuando comparado con miembro control, esos movimientos estaban similares excepto la abducción, que aunque próximo a valores funcionales, aún era más pequeña que el miembro de control (p<0,029). En cuanto a la calidad de vida se encontró una mejora significativa en la función física (p=0,004), la reducción del dolor (p=0,046), dificultad financiera (p=0,008) y síntomas en el brazo (p=0,019). Conclusión: El abordaje fisioterapéutico ha mejorado la amplitud del movimiento y la calidad de vida de mujeres tras la cirugía para el cáncer de mama, pero seguimientos más largos pueden traer beneficios adicionales.  Introdução: As complicações apos a cirurgia para câncer de mama podem determinar prejuízos na amplitude de movimento do ombro e impacto negativo nas atividades do cotidiano, interferindo na qualidade de vida. Objetivo: Comparar a amplitude de movimento e a qualidade de vida antes e após dez sessões de fisioterapia no pós-operatório de cancer de mama. Método: Realizou-se um ensaio clínico autocontrolado, envolvendo 36 mulheres. A amplitude de movimento foi avaliada pela goniometria do ombro homolateral e contralateral (controle). A qualidade de vida foi avaliada pelos questionários European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C-30 (EORTC QLQ-C30) e Breast Cancer Module (BR-23). Utilizaram-se os testes t de Student dependente e independente, adotando-se p<0,05. Resultados: Após a décima sessão, encontrou-se melhora significativa da flexão (p<0,001), extensão (p<0,001), abdução (p<0,001), adução (p<0,010), rotação medial (p<0,011) e rotação lateral (p<0,020). Quando comparado com membro controle, esses movimentos estavam similares, exceto a abdução, que embora, próxima a valores funcionais, ainda estava menor do que o membro controle (p<0,029). Quanto a qualidade de vida, encontrou-se melhora significativa na função física (p=0,004), redução da dor (p=0,046), dificuldade financeira (p=0,008) e sintomas no braço (p=0,019).
