Pharyngocutaneous Fistula in Cancer Patients: Implications for Nursing

Resumo: Introduction: Cancer of the larynx is one of the most common to reach the head and neck, representing 25% of malignant tumors that affect this area. When diagnosed early, this cancer is curable, but in most cases patients are submitted to total laryngectomy and pharyngolaryngectomy, in which the most serious complication is pharyngocutaneous fistula. The present study addresses the main implications of both pharyngocutaneous fistula and nursing care in patients undergoing total laryngectomy. Objective: To describe the main implications of the pharyngocutaneous fistula complication to subsidize nursing care. Method: Study of an integrative literature review, whose data were collected in the Lilacs, Medline, Pubmed and Google Academic databases, of which four articles were selected, which were suitable for the criteria of this study. Results: As a result, we found out that all the authors emphasize that the pharyngocutaneous fistula is the largest and most frequent complication in patients who undergo total laryngectomy, with a variety of factors listed in the literature that result in fistula formation, and identification of these risk factors is essential for planning the peri-operative nursing care. Conclusion: There are few studies on the risk factors for the formation of pharyngocutaneous fistula and nursing care. The nurse should be involved in formal education to prevent this complication, ensure physiological recovery and rehabilitation of the patient, in addition to promote healing, decreasing risks of infection, length of stay, hospital costs and promote improved quality of life of these patients.

Cuándo el diagnóstico es precoz, esta neoplasia tiene cura, pero en la mayoría de los casos, los pacientes son sometidos a la faringo laringotomía y laringotomía total, en los cuales la complicación más grave es la fístula faringo cutánea. El presente estudio trata de las principales implicaciones de la fístula faringo cutánea y la atención en enfermería en los pacientes sometidos a la laringectomía total. Objetivo: Describir las principales implicaciones de la fístula faringo cutánea para subsidiar la asistencia de enfermería. Método: Se trata de un estudio de revisión integradora de la literatura, cuyos datos han sido recogidos en las bases de datos LILACS, Medline, PubMed y Google Académico, de los cuales han sido seleccionados cuatro artículos que cumplían los criterios de selección del estudio. Resultados: En los resultados se obtuvo que todos los autores enfatizan que la fístula faringo cutánea es la complicación más grande y más frecuente en pacientes sometidos a la laringectomía total, con una diversidad de factores que son descriptos en la literatura que resultan en la formación de esta fístula y la identificación de estos factores de riesgo es esencial para la planificación de la asistencia de enfermería peri operatoria. Conclusión: Hay escasez de estudios sobre los factores de riesgo para la formación de la fístula faringo cutánea y los cuidados de enfermería. El enfermero debe actuar en la enseñanza para la prevención de esta complicación, garantizar la recuperación fisiológica y rehabilitación del paciente, así como favorecer la cicatrización, disminuyendo riesgos de infección, tiempo de hospitalización, gastos hospitalarios y promocionar la mejoría de la calidad de vida de los pacientes.  Introdução: O câncer de laringe é um dos mais comuns a atingir região da cabeça e pescoço, representando 25% dos tumores malignos que acometem essa área. Quando diagnosticada precocemente, essa neoplasia tem cura; mas, na maioria dos casos, os pacientes são submetidos à faringolaringectomia e laringectomia total, nos quais a complicação mais grave é a fístula faringocutânea. O presente estudo trata das principais implicações da fístula faringocutânea e os cuidados de enfermagem em pacientes submetidos à laringectomia total. Objetivo: Descrever as principais implicações da complicação fístula faringocutânea para subsidiar a assistência de enfermagem. Método: Estudo de revisão integrativa da literatura, com coleta de dados nas bases Lilacs, Medline, Pubmed e Google acadêmico, dos quais foram selecionados quatro artigos que se adequavam aos critérios de seleção do estudo. Resultados: Como resultados, todos os autores enfatizam que a fístula faringocutânea é a maior, e mais frequente complicação em pacientes submetidos à laringectomia total, com uma diversidade de fatores elencados na literatura que resultam na formação dessa fístula; a identificação desses fatores de risco é imprescindível para o planejamento da assistência de enfermagem perioperatória. Conclusão: Há escassez de estudos sobre os fatores de risco para a formação da fístula faringocutânea e os cuidados de enfermagem.
