Resumo: The authors make a bibliographic revision and analyse the relationship cholelithiasis and primary cancer of the gallbladder, based on the experience in 50 cases from the Surgery Department (General Surgery Service ofthe Hospital Universitário Clementino Fraga Filho) of the Medical School of the Federal University of Rio de Janeiro, Brazil, in the period from March 1978 to June 1992. The average of the patients was 60.5 years, the female predominating (47 cases - 94%). They report a dose relationship between cholelithiasis and cancer of the gallbladder. Both affections coexisted in 34 cases (68%) of the patients, leading to the approval of the hypothesis that cholelithiasis constitutes the main factor in the etiopathogeny of vesicular cancer; therefore, it becomes imperative to procrastinate the surgical treatment of lithiasis of the gallbladder.

A idade média dos pacientes foi de 60,5 anos, predominando nas mulheres (47 casos - 94%). Relatam uma estreita relação entre colelitíase e câncer da vesícula biliar.
