Women with Breast Cancer: the Meanings of Social Issues During the Neoadjuvant Chemotherapy Treatment

Resumo: Introduction: Breast cancer is the second most common malignant neoplasm in women worldwide and the main tumor concerning to deaths of Brazilian women. This finding may be attributed to the high number of diagnosis in advanced stage. In this case, one of the available treatments is the neoadjuvant chemotherapy, which causes many side effects, including impacts in social life. Objective: To identify the meanings of social issues mentioned by female patients diagnosed with malignant breast cancer, during their neoadjuvant chemotherapy treatment at National Cancer Institute. Method: This is a qualitative study, based on the analysis and interpretation of “hermeneutic-dialectic”. There were conducted 10 semi-structured surveys with users with breast cancer who were enrolled at National Cancer Institute José Alencar Gomes da Silva (INCA) in 2012, and who underwent neoadjuvant chemotherapy. Results: The central role of women in family structures and the process of feminization of poverty were aspects that stood out in the study. Conclusion: It was realized that the reflections of the phenomena that affect women in the world nowadays are even more decisive when they have passed through a chemotherapy treatment for breast cancer. We believe that public policies that support these women are needed in order to enable them the access to comprehensive health care, once we understand that the guarantee of social and financial support are central to the main goal which is the promotion of health.

Esta constatación puede atribuirse al elevado número de diagnósticos en estadiamiento avanzado. En este caso, una de las propuestas de tratamiento es la quimioterapia neoadyuvante, que produce muchos efectos colaterales con repercusión en la vida social. Objetivo: identificar las expresiones de la cuestión social, relatadas por pacientes mujeres con diagnostico de neoplasia maligna de mama, durante el tratamiento de quimioterapia neoadyuvante en el Instituto Nacional del Cancer José Alencar Gomes da Silva (INCA). Método: Se trata de un estudio de naturaleza cualitativa, pautado en el método de análisis e interpretación de la “hermenéutica-dialéctica”. Han sido realizadas 10 entrevistas semiestructuradas con usuarias con cáncer de mama dadas de alta en el Instituto Nacional del Cáncer José Alencar Gomes da Silva (INCA) en 2012, que han realizado quimioterapia neoadyuvante. Resultados: La centralidad de la mujer en la estructura familiar y el proceso de feminización de la pobreza fueron aspectos que se destacaron en la investigación. Conclusión: Se ha notado que los reflejos de los fenómenos que afectan a las mujeres en el mundo actual son aún más contundentes cuando ellas pasan por un tratamiento de quimioterapia para el cáncer de mama. Se sugiere la necesidad de políticas públicas que apoyen a esas mujeres permitiéndoles el acceso a la atención integral a la salud, pues entendemos que la garantía del soporte social y financiero son factores fundamentales para el objetivo mayor de promocionar la salud.  Introdução: O câncer de mama é o segundo tipo de neoplasia maligna mais frequente em mulheres no mundo e o que causa mais óbitos em mulheres brasileiras. Essa constatação pode ser atribuída ao elevado número de diagnósticos em estadiamento avançado. Nesse caso, uma das propostas de tratamento e a quimioterapia neoadjuvante que causa muitos efeitos colaterais com repercussões na vida social. Objetivo: Identificar as expressões da questão social, referidas por pacientes mulheres com diagnóstico de neoplasia maligna de mama, durante o tratamento de quimioterapia neoadjuvante no Instituto Nacional de Câncer José Alencar Gomes da Silva (INCA). Método: Trata-se de um estudo de natureza qualitativa, pautado no método de análise e interpretação da “hermenêutica dialética”. Foram realizadas dez entrevistas semiestruturadas com usuárias com câncer de mama, matriculadas no INCA em 2012, que realizaram quimioterapia neoadjuvante. Resultados: A centralidade da mulher nos arranjos familiares e o processo de feminização da pobreza foram aspectos que se destacaram na investigação. Conclusão: Percebeu-se que os reflexos dos fenômenos que atingem as mulheres no mundo atual são ainda mais contundentes quando elas passam por um tratamento quimioterápico de câncer de mama.
