Resumo: Cervical cancer's incidence has been increasing in all social groups. In the last 20 years, it has been responsible for the hightest death rates among Brazilian women. There was an estimate of 21.725 new occurences with 6.815 deaths for 1998. The projection for 1997 forelaw an increase of 3% in relation to 1996. The object of this study are the gender relations expressed by the Client who has a spontaneous demand for Pap smear. The objective is to understand the impact of the construction of gender relations and its influence in the spontaneous demand to Pap Smear. The semi-structural interview was used to clients who went to the 15th Health Center, located in Salvador - Bahia. Using for analysis the referential of gender and cancer prevention was possible to identify five categories which express characteristics of the powerful relation between the professional and the client during the test and its dynamic. Assistencial programs don’t ponder on a dimension that values gender aspects, and despite the importance given to the exam by the clients, women don’t considered the test in the routine of cancer prevention.
