Modified Hemicorporectomy: Case Report

Resumo: Introduction: The squamous cell carcinoma is recognized as one of the late complications of pressure ulcers. Due to its aggressive biological behavior and the often late diagnosis, extensive local resections are usually necessary to control this disease. Objective: Describe a modified hemicorporectomy technique, preserving the pelvic bones in a patient with multicentric tumor, locally advanced. Material and Methods: We report a case of a 57-year-old male paraplegic since his teens, with decubitus ulcers in the sacral and trochanteric regions, complicated by squamous cell carcinoma, treated by a modified hemicorporectomy technique, preserving the pelvic bones. Results: The surgical procedure was performed by a multidisciplinary team, allowing complete resection of the tumor through hemicorporectomy, preserving the bones of the pelvis and lumbosacral spine. The surgery lasted eight hours, the patient stayed for three days in intensive care unit, total hospitalization time was fifteen days and there was no need for blood transfusion. In the immediate postoperative period, there was partial necrosis of the muscle cutaneous flap and the patient was discharged in good health. Conclusions: Despite his physical appearance drastically change and the high morbidity of treatment, the surgical procedure improved his quality of life and increased his chances of cure.

Debido a su comportamiento biológicamente agresivo y al retraso en el diagnóstico, generalmente son necesarias resecciones locales extensas para el control del tumor primario. Objetivo: Describir la hemicorporectomia modificada con la preservación de los huesos de la pelvis en un paciente con tumor multicéntrico, localmente avanzado. Material y Métodos: Presentamos el caso de un parapléjico varón de 57 años de edad, desde su adolescencia com úlceras por decúbito en las regiones sacras y t trocantéricas, complicadas con un carcinoma de células escamosas, tratados por hemicorporectomia modificada, con la preservación de los huesos pélvicos. Resultados: El procedimiento quirúrgico fue realizado por un equipo multidisciplinario, lo que permitió la resección completa del tumor a través de hemicorporectomia con preservación de los huesos de la pelvis y de la columna lumbosacra. La operación duró ocho horas, el paciente permaneció durante tres días en cuidados intensivos, el tiempo de hospitalización total fue de quince días y no hubo necesidad de transfusión de sangre. En el postoperatorio inmediato ocurrió necrosis parcial del colgajo musculo cutáneo y el paciente fue dado de alta en buenas condiciones. Conclusión: A pesar de que su apariencia física cambio drásticamente y de la morbilidad del tratamiento, el procedimiento quirúrgico proporcionó una mejor calidad de vida e hizo posible la cura de la enfermedad.  Introdução: O carcinoma de células escamosas é, reconhecidamente, uma das complicações tardias das úlceras de pressão. Devido ao seu comportamento biológico agressivo e ao diagnóstico frequentemente tardio, usualmente são necessárias ressecções locais extensas para controle desta afecção. Objetivo: Descrever a hemicorporectomia modificada com preservação dos ossos pélvicos em paciente com lesão tumoral multicêntrica, localmente avançada. Material e Métodos: Relatamos o caso de um paciente de 57 anos, sexo masculino, paraplégico desde a adolescência, apresentando úlceras de decúbito em regiões trocantéricas e sacral, complicando com carcinoma de células escamosas, tratado por meio de hemicorporectomia modificada, preservando os ossos pélvicos. Resultados: O procedimento cirúrgico foi realizado por equipe multidisciplinar, possibilitando a ressecção de toda a lesão tumoral, por meio de hemicorporectomia, preservando os ossos da pelve e coluna lombossacral. O ato operatório durou oito horas, o paciente permaneceu por três dias em terapia intensiva, o tempo total de internação foi 15 dias e não houve necessidade de hemotransfusão. No período pós-operatório mediato ocorreu necrose parcial do retalho cutâneo e o paciente recebeu alta hospitalar em boas condições.
