Quality of Life in Patients Suffering from Upper Aerodigestive Tract Cancer at a University Hospital

Resumo: Introduction: Cancers of the head and neck reach 5% of all diagnosed cases of cancer in the world. And this can affect the quality of life of the affected people, because the treatment is aggressive (mutilating) even years after treatment. Objective: To profile the people affected by cancer in the upper aerodigestive tract treated at Hospital Universitario Cassiano Antonio Moraes (HUCAM); to identify Quality of Life domains affected by the treatment for this cancer and relate the sociodemographic and clinical domains affected Quality of Life of patients surveyed. Method: for this study of quantitative approach, we used the following instruments: Inventory I with sociodemographic and clinical data from patients, the instruments of European Organization for Research and Treatment of Cancer (EORTC ) QLQ-C30 (version 3) and QLQ-H&N35. Results: Patients with three types of combined treatment and advanced stage presentd a lower Physical Function with statistical significance (p = 0.031 and p = 0.010 respectively). The Primary Location of Larynx had greater association with Global Health/Quality of Life, Role Performance and Social Function, with statistical significance (p = 0.037, p=0.045 and 0.047 respectively). Conclusion: the late diagnosis interferes in the choice of therapy, but is more common to use a combined, more agressive treatment causing numerous consequences which influence the quality of life. There is a need to know the domains that are being harmed in the creation of treatment for a proper approach to the Full Attention of those patients.

Y esto puede afectar a la calidad de vida de las personas afectadas, ya que el tratamiento es agresivo (mutilador), incluso anos despues del tratamiento. Objetivo: Trazar perfiles de las personas afectadas por el cáncer en el tracto aerodigestivo superior, atendidas en el Hospital Universitario Cassiano Antonio Moraes (HUCAM); identificar los dominios de calidad de vidas afectadas por el tratamiento para este tipo de cáncer y relacionar los datos sociodemográficos y clínicos con dominios afectados de la calidad de vida de los pacientes estudiados. Método: estudio de abordaje cuantitativo. Se han utilizado los instrumentos: Inventario I con datos sociodemográficos y clínicos de los pacientes, instrumentos de La Organización Europea de Investigación y Tratamiento del Cáncer (EORTC ) QLQ-C30 (versión 3) y QLQ-H&N35. Resultados: Los pacientes con tres tipos de tratamiento combinados y estadio avanzado presentaron una Función Física más pequeña con significancia estadística (p=0,031 y p=0,010 respectivamente). La Ubicación Primaria de la Laringe tuvo mayor asociación con Salud/Calidad de Vida Global, Desarrollo del papel y Función Social, con significancia estadística (p=0,037, p=0,045 y 0,047 respectivamente). Conclusión: El diagnóstico tardio interfiere en la elección de la terapia, siendo mas comun el uso del tratamiento combinado y mas agresivo, provocando numerosas consecuencias que influyen en la calidad de vida. Hay la necesidad de conocer los dominios que están siendo perjudicados en la realización del tratamiento para un abordaje adecuado destinado a la Atención Integral de estos pacientes.Introdução: As neoplasias de cabeça e pescoço atingem 5% de todos os casos diagnosticados de câncer no mundo. Objetivo: Traçar o perfil das pessoas acometidas por câncer no trato aerodigestivo superior, atendidas no Hospital Universitário Cassiano Antônio Moraes (HUCAM); identificar os domínios de Qualidade de Vida afetados com o tratamento para este câncer; e relacionar os dados sociodemográficos e clínicos com domínios afetados de Qualidade de Vida dos pacientes pesquisados. Método: Estudo de abordagem quantitativa. Utilizaram-se os instrumentos: Inventário I com dados sociodemográficos e clínicos dos pacientes, instrumentos da Organização Europeia para Pesquisa e o Tratamento do Câncer (EORTC ) QLQ-C30 (versão 3) e QLQ-H&N35. Resultados: Os pacientes com três tipos de tratamento combinado e estádio avançado apresentaram função física menor com significância estatística (p=0,031 e p=0,010 respectivamente). A localização primária da laringe teve maior associação com Saúde/Qualidade de Vida Global, desempenho de papel e função social, com significancia estatística (p=0,037, p=0,045 e 0,047, respectivamente). Conclusão: O diagnóstico tardio interfere na escolha da terapêutica, sendo mais comum o uso de tratamento combinado e mais agressivo, acarretando inúmeras consequências que influenciam na Qualidade de Vida.
