O Controle da Dor em Pacientes Portadores de Câncer Avançado: um Problema Nacional Negligenciado

Resumo: Many advanced cancer patients suffer from chronic and severe pain, even after the use of the modem tools of chemotherapy, radiotherapy and neurosurgery for pain control, and they need narcotic analgesics in adequate doses and at regular intervals for a dignified life relatively pain free. The fear of patients becoming dependent on narcotics is unfounded in the contexto of terminal illnes. In general, physicians have an inadequate knowledge ou pain control. In our Country, there only are two types of narcotics (morphine and meperidine) and only as injectables. The atitude of our governamental authorities has been exclusively as of police. The problem of the Street addict interferes with the one suffering from advanced cancer, what is not fair. The sale and distribuition of narcotics are complex and difficult. We must have oral narcotics in the market, for example Methadone, hydromorphone tablets (Dilaudid) and liquid oral solution of morphine or hydromorphone. We also need suppositories of morphine and more potent injectables. In thepast years, several narcotics were withdrawn from the market, most likely due to the illegal abuse, leaving the pain patient without adequate relief. Unfortunately, a large number of patients spends their last days in excrutiating pain in our Country. It is a social, moral and ethical obligation of the medical profession and government to ease this problem.

O medo de tornar estes pacientes dependentes de narcóticos não tem fundamento no contexto de doença terminai Os médicos em geral têm um conhecimento inadequado sobre o controle da dor. Em nosso país existem apenas dois tipos de narcóticos (morfina e petidina) e somente injetáveis. A atitude de nossas autoridades governamentais tem sido exclusivamente policial. O problema do viciado de rua interfere com quem sofre de câncer avançado, o que não é justo. A venda e distribuição de narcóticos são complexas e difíceis. Temos que ter no comércio narcóticos orais como por exemplo a Metadona, hidromorfona comprimidos (Dilaudid) e solução líquida oral de morfina ou hidromorfona. Também necessitamos de morfina supositórios e injetáveis mais potentes. Nos últimos anos, vários narcóticos foram retirados do mercado, provavelmente devido ao abuso ilícito, ficando os pacientes com dor sem alívio adequado. Infelizmente, um grande número de pacientes passam seus últimos dias com dor excruciante, em nosso país.
