Breast Cancer in Young Women: Analysis of 12.689 Cases

Resumo: Introduction: Breast cancer in women under 40 years old is unusual. However, in this population the disease usually progresses with worse prognosis. Objectives: To describe the clinical and epidemiological profile of young women diagnosed with breast cancer in Brazil and to compare the clinical characteristics among women aged less than 34 years and those between 35 and 39 years. Method: Cross-sectional study of analytic cases of breast cancer in women aged 18-39 years, enrolled in the Integrator Module of Cancer Hospital Records and Cancer Hospital Registry of the State of São Paulo, from 2000 to 2009. It was performed a descriptive analysis of the variables. Results: There were included 12.689 cases. The median age was 36 years, most of women went to High School (32.3%) and came from the National Health System (74.6%). The advanced stage (≥ IIB) was recorded in 62.8% of cases and at the end of the first treatment, 44.4% of patients had no evidence of disease. Very young women presented more frequently a tumor size > 2 cm, positive lymph node status, metastasis, advanced clinical stages (≥ IIB) and lack of therapeutic response to the first treatment. Conclusion: In Brazil, young women are in advanced stage when they are diagnosed with breast cancer. Those too young (< 35 years) have a more advanced disease stage and poorer response to treatment compared to those aged 35-39.

Pero, en esa población la enfermedad camina, en general, con peor pronóstico. Objetivos: Describir el perfil clínico y epidemiológico de mujeres jóvenes diagnosticadas con cáncer de mama en Brasil y comparar las características clínicas entre mujeres con edad inferior a los 34 años y aquellas entre 35 y 39 años. Método: Estudio transversal de los casos analíticos del cáncer de mama en mujeres entre los 18 y 39 años, inseridos en el Módulo Integrador de los Registros Hospitalarios del Cáncer y en el Registro Hospitalario del Cáncer del Estado de São Paulo, entre los años de 2000 y 2009. Fue realizado el análisis descriptivo de las variables. Resultados: Han sido incluidos 12.689 casos clínicos. El promedio de edad fue de 36 años, la mayoría de las mujeres poseía educación secundaria obligatoria (32,3%) y procedentes del Sistema Nacional de Salud (74,6%). El estadiamiento avanzado (≥ IIB) fue registrado en 62,8% de los casos y, al final del primero tratamiento médico, 44,4% de las pacientes se encuentran sin evidencia de la enfermedad. Mujeres muy jóvenes presentaron con más frecuencia tamano del tumor >2cm, estatus de los ganglios linfáticos positivo, presencia de metastasis, estadiamiento clínico avanzado (≥IIB) y ausencia de respuesta terapéutica al primero tratamiento médico. Conclusión: En Brasil mujeres jóvenes con cáncer de mama presentan estadiamiento avanzado al diagnóstico. Aquellas muy jóvenes (< 35 años) presentan la enfermedad aun más avanzada y peor respuesta terapêutica que aquellas entre 35 y 39 años.Introdução: O câncer de mama em mulheres com idade inferior a 40 anos e incomum. A idade mediana foi de 36 anos, a maioria das mulheres possuía ensino médio (32,3%) e era proveniente do Sistema Único de Saúde (74,6%). O estadiamento avançado (≥IIB) foi registrado em 62,8% dos casos e, ao final do primeiro tratamento, 44,4% das pacientes encontravam-se sem evidencia da doença. Mulheres muito jovens apresentaram mais frequentemente tamanho do tumor >2cm, status dos linfonodos positivo, presença de metástase, estadiamento clínico avançado (≥IIB) e ausência de resposta terapêutica ao primeiro tratamento. Conclusão: No Brasil, mulheres jovens com câncer de mama apresentam estadiamento avançado ao diagnostico.
