Nutritional Satus and Inflammatory Markers of Preoperative Oral Cavity and Oropharynx Cancer Patients

Resumo: This article aimed to characterize patients with oral cavity and oropharynx cancer as to their protein-energy nutritional state and presence of inflammatory markers. Thus, a case series was conducted with 48 patients who were consecutively sent to surgery without any previous treatment. Their nutritional state was analyzed using laboratory, anthropometric, dietary and clinical indicators. Inflammatory markers were determined by alpha 1- acid glycoprotein and C-reactive protein. There was weight loss >5% in 31% of the patients within the last six months. The body mass index detected malnutrition in 14% of the patients. The adequacy level of energetic intake was 61±25 according to nutritional recommendations. According to subjective global assessment, 37% of the patients were malnourished. There were low serum concentrations of hemoglobin in 48% of patients, albumin in 44%, retinol-binding protein in 33%, total lymphocyte count in 25%, transthyretin in 14%. On the other hand high concentrations of alpha 1-acid glycoprotein were found in 4% of patients and C-reactive protein in 10%. Laboratory protein indicators (hemoglobin and albumin) and the subjective global assessment showed higher prevalence of nutritional deficit when compared to other indicator studied. The occurrence of patients with systemic inflammatory response was small, thus low serum proteins suggest malnutrition. Among the anthropometric indicators, the percentage of weight loss as to previous body weight seems to be a more appropriate indicator of changes in body composition than the body mass index.

El estado nutricional fue evaluado a través de indicadores clínicos, dietéticos, antropométricos y de laboratorios. La actividad inflamatoria fue determinada por medio de alfa-1-glicoproteína ácida y proteína-C-reactiva. Se ha comprobado una pérdida de peso >5% en 31% de los pacientes durante los últimos seis meses. La desnutrición diagnosticada por el índice de masa corporal fue encontrada sólo en 14% de los pacientes. La adecuación porcentual de la ingesta energética fue 61±25, según las recomendaciones nutricionales. A través de la evaluación nutricional subjetiva, el 37% de los pacientes sufrían de desnutrición. Se encontraron concentraciones séricas disminuidas de hemoglobina en 48%, albúmina en 44% y proteína transportadora de retinol en un 33%. Por otra parte, se observaron concentraciones aumentadas de alfa-1-glucoproteína ácida en 4% y de proteína-Creactiva en 10% de los pacientes. Los indicadores de laboratorio proteicos (hemoglobina y albúmina), y la evaluación nutricional subjetiva, mostraron una mayor prevalencia de desnutrición en relación con otros indicadores estudiados. La frecuencia de pacientes con actividad inflamatoria fue pequeña, por lo que la disminución de la albúmina y demás proteínas séricas sugiere la malnutrición. Desnutrição diagnosticada pelo índice de massa corporal foi encontrada em somente 14% dos pacientes. A adequação percentual da ingestão energética foi 61±25 segundo as recomendações nutricionais. Pela avaliação nutricional subjetiva, 37% dos pacientes estavam desnutridos. Encontraram-se concentrações séricas diminuídas de hemoglobina em 48%, albumina em 44% e proteína ligadora do retinol em 33%. Por outro lado, observaram-se concentrações aumentadas de alfa-1-glicoproteína ácida em 4% e de proteína-C-reativa em 10% dos pacientes. Os indicadores laboratoriais proteicos (hemoglobina e albumina) e a avaliação nutricional subjetiva mostraram maior prevalência de déficit nutricional em relação aos outros indicadores estudados. A frequência de pacientes com atividade inflamatória foi pequena, de modo que a diminuição da albumina e das demais proteínas séricas sugere desnutrição.
