Evaluation of Uterine Cervical Cancer Screening Indicators in the Amazonas, North of Brazil, from 2001 to 2005

Resumo: The study aimed to evaluate the uterine cervical cancer screening indicators in the Amazonas, Brazil, from 2001 to 2005. It was a descriptive study, with ecological and time-based analysis of the methods used by the program, using IBGE and SISCOLO/DATASUS as database. The data were stored at software Excel and analyzed at Software Epi-Info. The target-population coverage rate proportion varied from 75.78% to 78.07%; the first time exams fulfillment oscillated from 8.89% to 27.17%. Results considered within normality were 1.86% in 2001 and 2.23% in 2005; atypical cells varied between 5.13% and 1.00% and unsatisfactory samples varied between 4.3% and 3.45%. The ratio between low grade-Cervical Intraepithelial Neoplasia (CIN) and high grade-CIN scored 8.17 and 6.83. The target-population exams concentration rate has scored similarly to the national standard, though it is still below developed countries standards. The percentage of population who informed being examined for the first time has kept stable; however, a reduction in the number of answers "does not know" and an increase in the number of positive answers for previous examination imply a good performance of the program accessibility and information. Mass screening indicators showed elevation of normality-edge exams and atypical cells reduction. The ratio between low grade-CIN and high grade-CIN showed a tendency to decrease, pointing out a low grade-SIL decrease and high grade-CIN stability, demonstrating that measures are necessary to change the precursory lesions profile.

Un estudio descriptivo, con análisis ecológico y temporal de los métodos utilizados por el programa, teniendo como fuente de datos el IBGE y el SISCOLO/DATASUS. Los datos fueron almacenados en el programa Excel y analizados en el programa Epi-Info. La tasa de cobertura de la población meta osciló entre 75,78% y 78,07%; la realización de exámenes, por primera vez, osciló entre 8,89% y 27,17%. Los resultados dentro de la normalidad fueron de 1,86% en 2001 y 2,23% en 2005, las atipias oscilaron entre 5,13% y 1,00% y las muestras insatisfactorias entre 4,30% y 3,45%. La relación entre las lesiones precursoras de bajo grado (LBG) y las lesiones precursoras de alto grado (GAL) fue de 8,17 y 6,83. La tasa de concentración de exámenes de la población meta se encuentra de acuerdo con el estándar nacional, pero aún por debajo de los estándares de los países desarrollados; el porcentual de esta población que declaró haber realizado el examen por primera vez se mantuvo estable, sin embargo la reducción de la respuesta "no sabe" y el aumento de la respuesta positiva para el examen anterior insinúa el buen desempeño del programa en cuanto a la accesibilidad e información; los indicadores de rastreo mostraron una elevación de exámenes en el límite de la normalidad y reducción de las atipias celulares. La relación entre las LBG y las LAG mostró tendencia hacia la caída, destacándose una disminución de las LBG y una estabilidad de las LAG, demostrando que son necesarias medidas para el cambio de perfil de las lesiones precursoras.O estudo teve por objetivo avaliar os indicadores de rastreamento do câncer do colo do útero no Amazonas, no período de 2001 a 2005. A taxa de concentração de exames da população-alvo encontra-se de acordo com o padrão nacional, porém ainda abaixo dos padrões dos países desenvolvidos; o percentual dessa população que informou ter realizado o exame pela primeira vez manteve-se estável, contudo a redução da resposta "não sabe" e a elevação da resposta positiva para exame anterior insinua a boa performance do programa quanto à acessibilidade e informação; os indicadores de rastreamento mostraram elevação de exames no limite da normalidade e redução das atipias celulares.
