Valor do Antígeno Carcinoembriônico no Acompanhamento das Neoplasias Malignas do Trato Gastrentérico

Resumo: The role of sequential determination of CEA levels in patients with malignant tumors of the G. I. tract was evaluated. Sixty one patients had their history reviewed: 22 beared rectal lesions (36,06%), 18 had large bowel lesions (29,54%), eight had gastric tumors (13,11%), five had sigmoid lesions (8,19%), four had anal canal lesions (6,55%) and four had cecum lesions (6,55%). High levels of CEA and the presence of tumor were associated in 63% of the patients, and in 60,6% of those with metastatic disease high levels of CEA were also found. Adenocarcinoma was the most common histologic type found and CEA levels were high in all well differentiated tumors. High levels of CEA correlated better with the presence of metastatic disease than did liver scintigraphy. This was made clear by the findings of abnormal levels of CEA and normal scintigraphy. Survival of patients with normal preoperative levels of CEA was 100%. Of those patients with abnormal preoperative CEA levels, only 40% were alive af ter 36 months. The possible interferences in serum dosages of CEA and its interpretation, the importance of measuring the tumor marker in tissues, and the need of establishing preoperative follow-up protocols for the better assesment of the CEA dosages and the better evaluation of malignant G. I. tract tumors are stressed.

I. tract tumors are stressed.A importância da dosagem de antígeno carcinoembriônico (CEA) nos tumores malignos do trato gastrentérico foi avaliada. Foram estudados 61 pacientes sendo 22 pacientes portadores de carcinoma de reto (36,0611.), 18 de cólon (29,54%), oito de estômago (13,1 1%), cinco de sigmóide (8,19%), quatro de canal anal (6,55%) e quatro de ceco (6,55%). A correlação entre dosagem elevada de CEA e presença de tumor foi de 63,6% e na presença de metástases foi de 60,6%. O tipo histológico mais encontrado foi o adenocarcinoma e o CEA esteve elevado na totalidade dos casos de adenocarcinomas bem diferenciados. A dosagem de CEA mostrou-se mais sensível em detectar metástases do que a cintilografia hepática, o que se evidenciou pela presença de cintilografia normal com elevação dos níveis de CEA. A sobrevida foi de 100% nos pacientes que apresentavam CEA normal no pré-operatório; dos que apresentavam níveis elevados de CEA, apenas 40% estavam vivos após 16 meses.
