Impact of Nutritional Status and Muscle Strength on Global Health and Quality of Life Status in Gastrointestinal and Lung Cancer Patients

Resumo: Introduction: Malnutrition is a frequent problem for cancer patients, interfering with their function and quality of life. Objectives: To evaluate the impact of nutritional status and muscle strength on quality of life noticed by gastrointestinal and lung cancer patients suitable to chemotherapy. Method: A cross-sectional study conducted in a chemotherapy service in southern Brazil. Patients had their nutritional status assessed by Body Mass Index and Subjective Global Assessment, muscle function assessed by handgrip strength and the Global Health and Quality of Life Status assessed by the European Organization for Research and Treatment of Cancer – Quality of Life Questionnaire Core-30. Data were analyzed by Stata 11.1™ statistic package. Results: Most patients were men (57.1%), and mean age was 63.9 ±22.6 years old. According to Body Mass Index, about 60% of the sample were eutrophic, but the Subjective Global Assessment found only 13% of the patients were well nourished. The mean quality of life score was 68 ±21.3. The non-dominant handgrip strength mean was 25.7 ±10.1Kgf, and it was not associated to quality of life (p=0.3). The worst nutritional status was associated to poorer quality of life, both assessed by Body Mass Index (p=0.04) or Subjective Global Assessment (p=0.007). Conclusion: There was high malnutrition prevalence according to Subjective Global Assessment and the nutritional status had significant impact on these patients’ Global Health and Quality of Life Status, but muscle strength was not associated.

Objetivos: Evaluar el impacto del estado nutricional y de la fuerza muscular en la calidad de vida de los pacientes com cáncer de tracto gastrointestinal y de pulmón con indicacion quimioterápica. Método: Estudio transversal realizado en un ambulatorio de quimioterapia en el sur de Brasil. Los pacientes han tenido su estado nutricional evaluado por el Índice de Masa Corporal y por la Evaluación Global Subjetiva. La función muscular fue evaluada por dinamometria y el Estado General de Salud y Calidad de Vida fue evaluado por European Organization for Research and Treatment of Cancer – Quality of Life Questionnaire Core-30. Los datos fueron analizados utilizando el paquete estadístico StataR 11.1. Resultados: La mayoría de los pacientes eran hombres (57,1%) y la media de edad fue 63,9 ± 22,6 años. Los tumores del tracto gastrointestinal han sido los más prevalentes (74%). Según el Índice de Masa Corporal, 60% de la muestra estaba eutrófica, pero la Evaluación Global Subjetiva encontró sólo 13% bien nutridos. La puntuación de calidad de vida promedia fue 68 ± 21,3. El promedio de la prensión de mano fue 25,7 ± 10,1 Kgf, no estando sus valores asociados con calidad de vida (p=0,3). El peor estado nutricional se asoció con puntuaciones de calidad de vida más bajas, tanto por el Índice de Masa Corporal (p=0,04) cuanto por la Evaluación Global Subjetiva (p=0,007). Conclusión: Hubo elevada prevalencia de desnutrición y el estado nutricional tuvo impacto significativo en el Estado de Salud Global y Calidad de Vida, pero no se encontró asociación con la fuerza muscular.  Introdução: A desnutrição e frequente em pacientes oncológicos, interferindo na funcionalidade e na qualidade de vida. Objetivos: Avaliar o impacto do estado nutricional e da força muscular na qualidade de vida de pacientes com câncer do trato gastrintestinal e de pulmão indicados a quimioterapia. Método: Estudo transversal em um serviço de quimioterapia do Sul do Brasil. Os pacientes tiveram o estado nutricional avaliado pelo Índice de Massa Corporal e pela Avaliação Subjetiva Global, a funcao muscular avaliada por dinamometria; e o domínio de Estado de Saude Geral e Qualidade de Vida avaliado pelo questionário European Organization for Research and Treatment of Cancer – Quality of Life Questionnaire Core-30. Os dados foram analisados no pacote estatístico Stata 11.1R. Resultados: A maioria dos pacientes era homem (57,1%) e a idade media foi 63,9 ±22,6 anos. Tumores de trato gastrintestinal foram mais prevalentes (74%). Segundo o Índice de Massa Corporal, 60% da amostra estava eutrófica; porém a Avaliação Subjetiva Global encontrou somente 13% bem nutridos. O escore de qualidade de vida medio foi 68 ±21,3. A média da forca de preensão manual não dominante foi 25,7 ±10,1Kgf, sem associação com qualidade de vida (p=0,3). O pior estado nutricional associou-se com menores escores de qualidade de vida tanto pelo Índice de Massa Corporal (p=0,04) quanto pela Avaliação Subjetiva Global (p=0,007).
