Detecção Imuno-Histoquimica de Antígenos de Superfície ABH em Carcinoma de Células Transicionais da Bexiga: Descrição do método e comparação com a histologia clássica

Resumo: | Deletion of ABH surface antigens is associated with malignant cell transformation in a variety of human neoplasms, including bladder carcinomas. Data from studies using the red cell adherence test (SRCA) suggested that iossof ABH antigens was associated with histologically high graded tumors, with a worse clinical outcome. However, the impact of these findings was attenuated due to the drawbacks of the technique. Based on the extraordinary accuracy of immunohistochemistry, the present study was performed using a Ulex europaeus lectin to detect ABH antigens, with ampiification mediated by the avidin-biotin-peroxidase complex. The main goals were the evaluation of the applicability of this method and the correlation of the presence of antigens with histoiogicai grading. Thirty-nine (59,1%) out of 66 specimens evaluated were positive for ABH antigens. 85% of grade I tumors were positive, and "strong" staining was achieved in 69,2%. Grade II tumors showed 82,7%, of positivity and this was “strong" only in 58,6%. On the other hand, only 17,0% high-graded lesions (grade III) were positive and, in this group "strong" staining was obtained only in 12,5%. Our results showed that this new technique overcame the drawbacks of the red cell adherence test and its quite applicable to formalin-fixed paraffin embedded tissues. This technique, together with other prognostic parameters, can be very heipfui in the elaboration of therapeutic strategy of patients with transitional cell carcinoma of the bladder. |

Estudos empregando técnica de aderência de hemácias (SRCA) sugerem que esta perda é mais significativa nas neoplasias menos diferenciadas, com pior prognóstico. Entretanto, o impacto destes achados em termos práticos foi atenuado por limitações metodológicas do SRCA.Sabendo da elevada eficácia dos métodos imuno-histoquímicos mais modernos, recorremos à lectina do Ulex europaeus para a detecção dos antígenos ABH, mediante amplificação com o complexo avidina-biotina-peroxidase, pesquisando correlação coma graduação histológica, critério clássico na avaliação prognóstica. Dos 66 casos estudados, 39 (59,1%) apresentavam manutenção dos antígenos em suas membranas. Nos casos grau I a positividade atingiu 85,0%, sendo quantificada como "intensa" em 69,2%. Nos casos graus II a positividade foi 82,7%, mas só em 58,6%, mostrou-se "intensa". No outro extremo, apenas 17j0%o dos casos grau III mostraram positividade, que foi "intensa" em 12,5%.
