Fallopian Tube Adenocarcinoma: a Report of Two Cases

Resumo: Primary malignant tumors in the fallopian tube are the least common in the female gynecologic apparatus, corresponding from 0.1 to 0.5% of the total of such tumors. The incidence peak is around 60 years. Very rigorous criteria must be applied in order to consider a primary neoplastic injury of the tube, because the closeness to both endometrial mucosa and ovary complicates the diagnosis. The first case is a 44-year-old female patient who underwent laparotomy for presenting a pelvic tumor. The right fallopian tube was identified with an injury and total hysterectomy with right salpingectomy and left oophorectomy, preserving the right ovary, were performed. The histopathological study showed a little differentiated adenocarcinoma in the right fallopian tube, as well as the presence of two metastatic focuses in the left ovary. Adjuvant chemotherapy was initiated. After six cycles, injuries in both pelvic and para-aortic were noticed, as well as injury of the vaginal cupola. The patient underwent a right oophorectomy and pelvic and para-aortic lymphadenectomy. The histopathological study of specimens did not show any neoplasm. The second case is a 63-year-old female patient with a cystic injury in the left uterine annex and a mixed injury in the right annex. A bilateral salpingo-oophorectomy was performed. The histopathological study showed a moderately differentiated adenocarcinoma in the right fallopian tube. After this diagnosis, hysterectomy and omentectomy were both performed and did not present neoplastic disease in the histopathology. Both patients are healthy and with no evidence of neoplastic disease in activity, 40 and 72 months after the first surgery, respectively.

El pico de incidencia es de alrededor de los 60 años. Criterios muy rigurosos deben ser aplicados para considerar una lesión neoplásica primaria de la tuba, ya que la proximidad de la mucosa del endometrio y del ovario dificulta el diagnóstico. El primero caso es de una paciente de 44 años de edad que realizó una laparotomía por presentar un tumor pélvico. Fue identificada una masa en la tuba uterina derecha y fue realizada una histerectomía total con salpingectomía derecha y ooforectomía izquierda, preservando el ovario derecho. La histopatología correspondió a adenocarcinoma poco diferenciado en la tuba derecha, así como la presencia de dos focos metastáticos en el ovario izquierdo. Quimioterapia adyuvante fue realizada. Después de seis ciclos, lesiones pélvicas y paraaórticas fueron observadas, además de lesiones de la cúpula vaginal. La paciente fue sometida a una ooforectomía derecha y linfadenectomia pélvica e paraaórtica y no hubo enfermedad neoplásica en el estúdio histológico de los especímenes. El segundo caso es de una paciente de 63 años de edad con una lesión quística en anexo uterino izquierdo y mixta en el anexo derecho. Fue sometida a una cirugía realizando una salpingooforectomía bilateral. La histopatología correspondió a adenocarcinoma de tuba moderadamente diferenciado en tuba derecha. Tras ese diagnóstico, histerectomía y omenctomía fueron realizadas, y no presentaron enfermedad en el histopatológico. Ambas pacientes están bien y sin evidencia de enfermedad neoplásica en actividad 40 y 72 meses después de la primera cirugía.Os tumores malignos primários de tuba uterina são os menos frequentes do aparelho genital feminino, correspondendo de 0,1 a 0,5% do total desses tumores. Devem-se aplicar critérios muito rigorosos para considerar uma lesão neoplásica primária da tuba, pois a proximidade da mucosa endometrial e do ovário dificulta o diagnóstico. O primeiro caso é de uma paciente de 44 anos de idade que se submeteu à laparatomia por apresentar um tumor pélvico. Identificou-se uma massa na tuba uterina direita, realizando-se histerectomia total com salpingectomia direita, ooforectomia esquerda, preservando-se o ovário direito. O histopatológico demonstrou adenocarcinoma pouco diferenciado na tuba direita, bem como a presença de dois focos metastáticos no ovário esquerdo. Iniciou-se quimioterapia adjuvante. Após seis ciclos, notaram-se lesões pélvica e para-aórtica, além de lesão de cúpula vaginal. A paciente submeteu-se à ooforectomia direita e linfonodectomia pélvica e para-aórtica, não havendo doença neoplásica no estudo histológico dos espécimes. O segundo caso é de uma paciente de 63 anos de idade com uma lesão cística em anexo uterino esquerdo e mista em anexo direito. Submeteu-se a uma cirurgia sendo realizada salpingooforectomia bilateral. Ao histopatológico, evidenciou-se adenocarcinoma de tuba moderadamente diferenciado em tuba direita. Após esse diagnóstico, fez-se histerectomia e omentectomia, que não apresentaram doença no histopatológico.
