The Prevalence of Scoliosis in Patients Diagnosed with Wilms’ Tumor with Follow-up free of Disease

Resumo: | Introduction: Wilms’ tumor is the most common malignant renal tumor in children. The treatment combines surgery, chemotherapy and, in some cases, radiotherapy. The musculoskeletal abnormalities have been described as the most frequent late effects related to the radiotherapy treatment. Objective: To verify the prevalence of scoliosis in patients diagnosed with Wilms’ tumor, treated with chemotherapy, and with or without radiotherapy after tumor resection, with at least 2 years of follow-up free of disease. Method: Cross-sectional study with 20 patients. Nine of those 20 patients had tumor resection surgery and received chemotherapy (CQ group) and 11 patients had tumor resection surgery and received both chemotherapy and radiotherapy (CQR group). All patients underwent a physical evaluation and a postural evaluation through computer-assisted photogrammetry. Results: Scoliosis was observed in 75% of the sample, 67% of the CQ group and 82% of the CQR group. The main postural deviations found during the physical evaluation were horizontal asymmetry of the shoulders and anterior pelvic tilt. The main postural deviations found through the computer-assisted photogrammetry evaluation were anterior pelvic tilt and angle between acromions, anterior superior iliac spines different from 0o, asymmetric horizontal alignment of the scapulae, asymmetric horizontal alignment of the pelvis and horizontal asymmetry of the scapulae in relation to T3. Conclusion: Patients diagnosed with Wilms’ tumor, treated with or without radiotherapy after tumor resection, with at least 2 years of follow-up free of disease present a high prevalence of musculoskeletal deviations, especially those related to developing scoliosis. |

Los daños musculo-esqueléticos han sido descritos como los efectos tardíos y secuelas más frecuentes del tratamiento de radioterapia. Objetivo: Conocer la prevalencia de la escoliosis em pacientes diagnosticados com tumor de Wilms, tratados con quimioterapia, con o sin radioterapia adyuvante después de la resección quirúrgica, fuera de tratamiento con seguimiento mínimo de 2 años. Método: Estudio observacional de corte transversal con 20 pacientes. Nueve recibieron solamente resección quirúrgica y quimioterapia (Grupo CQ), y 11 recibieron resección quirúrgica, quimioterapia y radioterapia (Grupo CQR). Todos los individuos fueron sometidos a evaluación física general y evaluación postural por fotogrametría computarizada. Resultados: La escoliosis se presentó em 75% de los casos, 67% del grupo CQ y 82% del grupo CQR. Las principales alteraciones observadas por la evaluación física fueron asimetría horizontal de los hombros y anteversión pélvica; y por la evaluación postural por fotogrametría computarizada fueron anteversión pélvica, ángulo entre los acromiones y las espinas ilíacas anterosuperiores diferente de 0o, asimetría del alineamiento horizontal de las escápulas, asimetría del alineamiento horizontal de la pelvis y asimetría horizontal de las escápulas en relación a T3. Conclusión: Los pacientes diagnosticados con Tumor de Wilms, tratados con o sin radioterapia adyuvante después de la resección quirúrgica, y fuera de tratamiento con seguimiento mínimo de 2 años presentan grande prevalencia de daños musculo-esqueléticos, principalmente el desarrollo de la escoliosis.Introdução: O tumor de Wilms é o tumor renal maligno mais comum na infância cujo tratamento combina cirurgia, quimioterapia e, em alguns casos, radioterapia. As alterações musculoesqueléticas têm sido descritas como os efeitos tardios e sequelas mais frequentes relacionados ao tratamento de radioterapia. Objetivo: Verificar a prevalência de escoliose em pacientes diagnosticados com tumor de Wilms, tratados com quimioterapia, e com ou sem radioterapia adjuvante após ressecção tumoral, fora de tratamento há pelo menos dois anos. Método: Estudo observacional transversal com 20 pacientes. Nove realizaram somente ressecção tumoral e quimioterapia (Grupo CQ) e 11 ressecção tumoral, quimioterapia e radioterapia (Grupo CQR). Todos os indivíduos da amostra foram submetidos à avaliação física geral e à avaliação postural por fotogrametria computadorizada. Resultados: A escoliose foi observada em 75% da amostra, 67% do grupo CQ e 82% do grupo CQR. Na avaliação física, as principais alterações encontradas no total da amostra foram assimetria horizontal dos ombros e anteversão pélvica; na avaliação postural por fotogrametria, as principais foram anteversão pélvica, ângulo diferente de 0º entre acrômios e as espinhas ilíacas anterossuperiores, assimetria do alinhamento horizontal das escápulas, assimetria do alinhamento pélvico horizontal e assimetria horizontal das escápulas em relação à T3.
