Respiratory Muscle Strength and Thoracic Mobility in Children and Adolescents with Acute Leukemia and Healthy School Students

Resumo: | Introduction: Leukemias are pointed out as the most common child neoplasm and the treatment chosen for this disease is chemotherapy. Among others, chemotherapy causes, at the respiratory level, damage to the epithelial tissue, which may lead to pnemonitis or even lung fibrosis. Objective: To compare respiratory muscle strength and chest mobility between children and teenagers with acute leukemia under chemotherapy maintenance phase and healthy individuals. Method: The sample consisted of 48 participants divided into two groups (group A with 16 children and adolescents with acute leukemia and group B with 32 healthy school students). Chest cirtometry, manovacuometry and anthropometric evaluation of individuals were done. After the descriptive analysis, the Kolmogorov-Smirnov sample identified the variables with normal distribution. These data were analyzed by means of non-paired t’Student and the non-parametric data by the Mann-Whitney test. Significant level of p<0.05 was attributed. Results: There was a significant difference between Groups A and B in the maximal inspiratory pressures (p=0.01), axillary retraction (p=0.012), expansion (p=0.007) and xiphoid retraction (p=0.005) variables. Conclusion: The findings indicate that a deficit may occur in either chest mobility or respiratory muscle strength in children and adolescents during the maintenance phase of chemotherapy for acute leukemia. This may indicate the need to periodically monitor and evaluate the respiratory muscles of this population, which would allow early diagnosis and intervention on the evidence of damage to lung function. |

Introducción: Reciben relieve las leucemias como la neoplasia infantil más común, y el tratamiento elegido para esta enfermedad es la quimioterapia. Entre otras, la quimioterapia trae, a nivel de sistema respiratorio, perjuicios al tejido epitelial, los cuales pueden culminar en neumonía o aun en fibrosis pulmonar. Objetivo: Comparar la fuerza de los músculos respiratorios y la movilidad en el tórax en niños y adolescentes acometidos de leucemia aguda en la fase de mantenimiento de la quimioterapia y en individuos sanos. Método: La muestra consistió de 48 participantes divididos en dos grupos (grupo A formado por 16 niños y adolescentes acometidos de leucemia aguda; y grupo B formado por 32 estudiantes sanos). Se llevó a cabo la evaluación de la movilidad del tórax, se midieron las presiones inspiratoria y espiratoria máximas, además de la evaluación antropométrica de los sujetos. Tras el análisis descriptivo, la prueba de Kolmogorov-Smirnov identificó las variables con distribución normal, las cuales fueron comparadas con el test t’Student no apareado y a otros por el test de Mann-Whitney. Un nivel de significación de p<0,05 fue atribuido. Resultados: Hubo una diferencia significativa entre los grupos A y B en las variables presión inspiratoria máxima (p = 0,01), retracción axilar (p=0,012), expansión (p=0,007) y retracción del xifoides (p=0,005). Conclusión: Los hallazgos indican que puede haber un déficit en la movilidad del tórax y la fuerza muscular respiratoria en niños y adolescentes en la fase de mantenimiento de la quimioterapia para la leucemia aguda. Esto puede indicar la necesidad de vigilar y evaluar periódicamente los músculos respiratorios de esta población, lo que permitiría el diagnóstico y la intervención precoces en la evidencia de daño a la función pulmonar. Entre outras, a quimioterapia provoca, em nível do sistema respiratório, danos ao tecido epitelial, que podem culminar em pneumonite ou até mesmo em fibrose pulmonar. Objetivo: Comparar a força dos músculos respiratórios e a mobilidade da caixa torácica de crianças e adolescentes com leucemia aguda na fase de manutenção da quimioterapia e de indivíduos saudáveis. Método: A amostra foi composta por 48 participantes distribuídos em dois grupos (grupo A, formado por 16 crianças e adolescentes com leucemia aguda; e grupo B, formado por 32 escolares saudáveis). Foram realizadas cirtometria torácica, manovacuometria e avaliação antropométrica dos sujeitos. Posteriormente à análise descritiva, a prova de Kolmogorov-Smirnov identificou as variáveis com distribuição normal. Estas foram comparadas através do teste t’student não pareado e, as demais, através do teste de Mann-Whitney. Atribuiu-se nível de significância p<0,05. Resultados: Observou-se diferença significativa entre os grupos A e B nas variáveis pressões inspiratórias máximas (p=0,01), retração axilar (p=0,012), expansão (p=0,007) e retração xifoidiana (p=0,005). Conclusão: Os achados indicam que pode ocorrer um déficit na mobilidade torácica e na força dos músculos respiratórios em crianças e adolescentes na fase de manutenção da quimioterapia para a leucemia aguda. Isso pode apontar para a necessidade de avaliar e monitorar periodicamente a musculatura respiratória dessa população, o que permitiria diagnóstico e intervenção precoces na evidência de dano à função pulmonar.
