IMPORTANCE OF KNOWLEDGE ABOUT CONDUCT, CARE AND PREVENTION IN DENTAL TREATMENT IN PATIENTS WITH DOWN SYNDROME: a literature review

Resumo: Down Syndrome, also known as Trisomy 21, was recognized in 1866 and proven in 1958. Down Syndrome is characterized by incorrect chromosomal distribution, with 3 main chromosomal changes possible: Simple or standard trisomy. Translocation or Mosaicism. It is estimated that the incidence of Down Syndrome is 3% of the world population. Being one of the main causes of intellectual disability, patients with DS also have similar physical characteristics, in addition to the common presence of systemic changes and changes in the oral cavity. Dental care in patients with DS must be early, thus avoiding the worsening of oral changes. During dental care, it is extremely important that the dentist uses techniques that facilitate treatment, offering greater comfort for the patient. Techniques similar to those used in pediatric dentistry can be used. In addition to these techniques, other means can be used, such as, in more extreme cases, general anesthesia, which is not recommended for these syndromic patients due to some physical changes that generally make orotracheal intubation difficult; the use of conscious sedation through the administration of benzodiazepines or the inhalation of nitrous oxide gas, being considered a good resource, making it possizle to carry out simpler procedures such as prophylaxis to less complex surgeries.

El síndrome de Down se caracteriza por una distribución cromosómica incorrecta, con 3 cambios cromosómicos principales posibles: trisomía simple o estándar. Translocación o mosaicismo. Se estima que la incidencia del Síndrome de Down es del 3% de la población mundial. Al ser una de las principales causas de discapacidad intelectual, los pacientes con SD también tienen características físicas similares, además de la presencia común de cambios sistémicos y cambios en la cavidad oral. La atención odontológica en pacientes con SD debe ser precoz, evitando así el empeoramiento de los cambios bucales. Durante el cuidado odontológico, es sumamente importante que el dentista utilice técnicas que faciliten el tratamiento, ofreciendo mayor comodidad para el paciente. Se pueden utilizar técnicas similares a las utilizadas en odontopediatría. Además de estas técnicas, se pueden utilizar otros medios, como, en casos más extremos, la anestesia general, que no se recomienda para estos pacientes sindrómicos debido a algunos cambios físicos que generalmente dificultan la intubación orotraqueal; El uso de la sedación consciente mediante la administración de benzodiacepinas o la inhalación de gas óxido nitroso, siendo considerado un buen recurso, posibilitando la realización de procedimientos más sencillos como la profilaxis hasta cirugías menos complejas.A Síndrome de Down (SD), também conhecida por Trissomia do cromossomo 21, foi reconhecida no ano de 1866 e comprovada em 1958. A SD é caracterizada pela distribuição cromossômica errada, podendo ocorrer 3 principais alterações cromossômicas: A trissomia simples ou padrão, A translocação ou Mosaicismo. Estima-se que a incidência de Síndrome de Down seja de 3% da população mundial. Sendo uma das principais causas de deficiência intelectual, os pacientes com SD apresentam também características físicas semelhantes entre si, além de ser comum a presença de alterações sistêmicas e alterações na cavidade oral. Deste modo, o atendimento odontológico em pacientes com SD deve ser precoce, evitando assim o agravamento de alterações orais. O objetivo desta pesquisa foi descrever as características, manejo, cuidados, prevenção e condutas para a realização do tratamento odontológico em pacientes com SD. O trabalho desenvolveu-se através de uma revisão narrativa da literatura. Durante o atendimento odontológico é de extrema importância que o cirurgião dentista utilize técnicas que facilitem o tratamento, oferecendo maior conforto para o paciente, podem ser utilizadas técnicas semelhantes as empregadas no atendimento odontopediatra.
