UMBILICAL HERNIA IN PEDIATRICS: an integrative review

Resumo: | Umbilical hernia is a condition that occurs when an intracavitary organ or tissue occurs in a failure of the abdominal wall. It is a condition common to newborns with small umbilical defects, which resolve in the first weeks of birth. However, there are cases where the defect remains and needs surgical repair or follow-up. This is a systematic literature review, in which a survey of 177 articles was carried out, of which 12 articles were used. The inclusion criteria were articles dealing with umbilical hernia in children, in English, Spanish and Portuguese. At the beginning of the research, 177 articles were selected that fit the key words, of which 165 (93,2%), were excluded, which did not address umbilical hernias in children. This work reports the anatomy of the abdominal wall, the definition of hernia, the types of hernias, the risk factors for the onset of the pathology, and how the diagnosis of umbilical hernia in children is performed. Furthermore, this article exposes the surgical treatment of umbilical hernia in children, and the criteria used to choose surgical or conservative treatment. Understanding the steps and indications for surgical treatment of infant umbilical hernia is necessary to make the work more effective. Communication and integration between the multidisciplinary team is necessary in order to analyze the child's vulnerabilities and needs, and establish the best follow-up for him. |

Es una condición común a los recién nacidos con pequeños defectos umbilicales, que se resuelven en las primeras semanas de nacimiento. Sin embargo, hay casos en los que el defecto permanece y necesita reparación quirúrgica o seguimiento. Se trata de una revisión sistemática de la literatura, en la que se realizó una encuesta de 177 artículos, de los cuales se utilizaron 12 artículos. Los criterios de inclusión fueron los artículos sobre hernia umbilical en niños, en inglés, español y portugués. Al inicio de la investigación, se seleccionaron 177 artículos que se ajustaban a las palabras clave, de los cuales 165 (93,2%), fueron excluidos, que no abordaban las hernias umbilicales en niños. Este trabajo informa la anatomía de la pared abdominal, la definición de hernia, los tipos de hernias, los factores de riesgo para el inicio de la patología y cómo se realiza el diagnóstico de hernia umbilical en niños. Además, este artículo expone el tratamiento quirúrgico de la hernia umbilical en niños, y los criterios utilizados para elegir el tratamiento quirúrgicoo conservador. Comprender los pasos y las indicaciones para el tratamiento quirúrgico de la hernia umbilical infantil es necesario para que el trabajo sea más efectivo. La comunicación e integración entre el equipo multidisciplinario es necesaria para analizar las vulnerabilidades y necesidades del niño, y establecer el mejor seguimiento para él.A hérnia umbilical é uma patologia que ocorre quando um órgão ou um tecido intracavitário se eventra em uma falha da parede do abdome. É uma condição comum aos recém-nascidos pequenos defeitos umbilicais, que se resolvem nas primeiras semanas de nascimento. Contudo, existem casos em que o defeito permanece e precisa de reparo cirúrgico ou acompanhamento. Trata-se de uma revisão sistemática de literatura, em que foi realizado um levantamento de 177 artigos, dos quais utilizou-se 12 artigos. Os critérios de inclusão foram artigos que tratassem sobre hérnia umbilical na criança, na língua inglesa, espanhola e portuguesa. Ao início da pesquisa, foram selecionados 177 artigos que se enquadravam nas palavras chave, dos quais excluiu-se 165 (93,2%), que não abordavam sobre hérnias umbilicais na criança. Relata-se nesse trabalho a anatomia da parede abdominal, a definição de hérnia, quais os tipos de hérnias, os fatores de risco para o surgimento da patologia, e como se realiza o diagnóstico da hérnia umbilical em crianças. Ademais, expõem-se nesse artigo acerca do tratamento cirúrgico da hérnia umbilical em crianças, e os critérios utilizados para escolha do tratamento cirúrgico ou conservador. A compreensão acerca das etapas e indicações de tratamento cirúrgico da hérnia umbilical infantil, é necessária para tornar o trabalho mais efetivo.
