Pulp Regeneration: A new therapeutic option in immature definitive teeth

Resumo: Permanent teeth with open apices present a great challenge in cleaning and shaping root canals; in addition, they have a high probability for the risk of fractures due to the presence of thin or weakened walls in the root portion. Thus, pulp regeneration has emerged as a promising alternative for the treatment of these teeth, in order to control the infection of the root canal system, using minimum action of endodontic instruments as well as the use of abundant irrigation during biomechanical preparation. This technique consists of inducing bleeding in the apical region to form a clot with undifferentiated cells that, associated with growth factors, are capable of inducing tissue repair and neoformation. Therefore, the present work aims to carry out a literature review on pulp regeneration therapy, highlighting its clinical applicability, advantages, disadvantages, and care protocols. For this purpose, bibliographic references indexed on sites such as Google academic, PubMed and Scielo were used. Research indicates that pulp tissue revitalization promotes root development and thickening in immature permanent teeth with pulp necrosis. It is concluded that, although the pulp regeneration technique does not have a protocol considered gold standard, when it is well indicated, it can bring very satisfactory results for the patient. However, further studies still need to be carried out to verify the effectiveness of this new therapeutic proposal.

Dessa forma, a regeneração pulpar surgiu como uma alternativa promissora para tratamento desses dentes, com intuito de controlar a infecção do sistema de canais radiculares, utilizando o mínimo de ação dos instrumentos endodônticos assim como também o uso de irrigação abundante durante o preparo biomecânico. Essa técnica consiste em induzir um sangramento na região apical para formação de um coágulo com células indiferenciadas que associados a fatores de crescimento são capazes de induzir uma reparação e neoformação tecidual. Sendo assim, o presente trabalho tem por objetivo realizar uma revisão de literatura sobre a terapia de regeneração pulpar, destacando sua aplicabilidade clínica, vantagens, desvantagens, e protocolos de atendimento. Para isso, foram utilizadas referências bibliográficas indexadas em sites como Google acadêmico, PubMed e Scielo. As pesquisas apontam que a revitalização do tecido pulpar promove o desenvolvimento e espessamento radicular em caso de dentes permanentes imaturos com necrose pulpar. Conclui-se então que apesar da técnica de regeneração pulpar não ter um protocolo considerado padrão-ouro, quando esta for bem indicada pode trazer resultados muito satisfatórios para o paciente.
