Management of Drug Treatment for Diabetic Patients

Resumo: Objective: To describe the management of drug treatment for diabetic patients. Method: This is a descriptive study, with a quantitative approach and cross-sectional design. The study sample was made up of users monitored at a Basic Health Unit in the Federal District, with type I or II DM, using oral hypoglycemic agents and/or insulin. The selection was made by random, conventional and simple sampling. The data collection instrument used listed sociodemographic variables, clinical recommendations and protocols for patient safety and management of patients with DM. The data found were analyzed using descriptive statistics. Results: The sample consisted of 41 participants, predominantly female and with low education, with an average of 66.8 years of age. Regarding the clinical profile, all users had type II DM, with a predominance of multimorbidity, high cardiovascular risk and achievement of glycemic control goals. It was observed that 68.5% were on polypharmacy, with 31.5% using sulfonylureas and 36.6% using insulin. The biggest barriers to medication adherence were forgetfulness and difficulty in keeping schedules, with 61% and 53.7%, respectively. Regarding insulin therapy, the main inconsistencies found were: 86.7% regarding lack of knowledge about the validity of insulin after opening the bottle; 93.3% lack of record of the date the bottle was opened; 73.3% inadequate insulin transport; 93.3% of inappropriate disposal of needles/syringes; 80% of inadequate rotation of application sites; and 85.7% do not remove air from the pen with each application. Conclusion: The diabetic patients in this study have encountered several difficulties in managing drug treatment, which can increase human error. In this way, the need for strategies to identify and manage these difficulties and equitable monitoring of this population, in order to promote autonomy, therapeutic adherence and patient safety.

Método: Trata-se de um estudo descritivo, com abordagem quantitativa e delineamento transversal. A amostra do estudo foi composta por usuários acompanhados em uma Unidade Básica de Saúde do Distrito Federal, portadores de DM tipo I ou II, em uso de hipoglicemiantes orais e/ou insulinas. A seleção se deu por amostragem aleatória, convencional e simples. O instrumento de coleta de dados utilizado elencava variáveis sociodemográficas, recomendações clínicas e protocolos de segurança do paciente e do manejo do paciente com DM. Os dados encontrados foram analisados por meio de estatística descritiva. Resultados: A amostra foi composta por 41 participantes, com predominância do sexo feminino e baixa escolaridade, com média de 66,8 anos de idade. Quanto ao perfil clínico, todos os usuários eram portadores de DM tipo II, observando-se predominância de multimorbidade, alto risco cardiovascular alcance das metas de controle glicêmico. Observou-se que 68,5% estavam em polifarmácia, sendo 31,5% em uso de sulfonilureias e 36,6% de insulinas. As maiores barreiras para adesão medicamentosa foram o esquecimento e a dificuldade em cumprir horários, com 61% e 53,7%, respectivamente. Quanto a insulinoterapia, as principais inconsistências encontradas foram: 86,7% a respeito do desconhecimento sobre validade da insulina após a abertura do frasco; 93,3% da ausência de registro da data de abertura do frasco; 73,3% de transporte inadequado da insulina; 93,3% do descarte inadequado de agulhas/seringa; 80% do rodízio inadequado de locais de aplicação; e 85,7% não retiram o ar da caneta a cada aplicação. Conclusão: Os pacientes diabéticos deste estudo têm encontrado diversas dificuldades para manejar o tratamento medicamentoso, o que pode potencializar o erro humano.
