Resumo: The global prevalence of arterial hypertension and type II diabetes mellitus represents a considerable burden on public health, and the Unified Health System (SUS) plays a crucial role in addressing these diseases, focusing on health promotion and primary care. The aim of this study was to identify, through integrative research, what are the main challenges in nurses' performance when carrying out health promotion actions for patients with hypertension and type II diabetes mellitus. The integrative literature review was used as a methodology. Studies published in the last 10 years in indexed scientific journals that addressed primary care related to type II diabetes and hypertension were included. The results demonstrated that nurses, who play a central role in primary care, face challenges such as lack of resources and patients' adherence to healthy lifestyle changes. As a result obtained in the present work and responding to the general objective, it became clear that the high rate of chronic non-communicable diseases arises from several health factors, those related to type II diabetes mellitus and systemic arterial hypertension are multifaceted and interact in a complex way.
