Resumo: | Vancomycin is an antibiotic of the glycopeptide class and acts on the cell wall of bacteria, disrupting their synthesis, especially in species of methicillin-resistant Staphylococcus aureus (MRSA), coagulase-negative Staphylococcus and Entecococcus faecium. It is a drug that has a nephrotoxic potential, due to its narrow therapeutic window and depending on the patient's hemodynamic stability. The presence of the clinical pharmacist together with the multidisciplinary team is necessary and important. This pharmaceutical professional is responsible for several responsibilities, including pharmacotherapeutic monitoring, which, by adjusting therapeutic doses, avoids drug-related problems (DRP). The present study aims to carry out an integrative review of the literature on the pharmacotherapeutic monitoring of vancomycin in patients in intensive care units. 13 articles were found in the online databases, which met the research inclusion and exclusion criteria. The presence of a clinical pharmacist in intensive care units is extremely important. This professional will perform several duties, including pharmacotherapeutic monitoring for vancomycin, which will guarantee the optimization of therapeutic doses of this antibiotic and avoid the appearance of adverse and/or toxic events, as well as possible failures in drug therapy, which could worsen the patient's clinical status. |

Ele é um fármaco que apresenta um potencial nefrotóxico, devido ter uma janela terapêutica estreita e depender da estabilidade hemodinâmica do paciente. A presença do farmacêutico clínico juntamente com a equipe multiprofissional se faz necessária e importante. Pois, este profissional farmacêutico é o responsável por várias atribuições, dentre as quais, a monitorização farmacoterapêutica, que através do ajuste nas doses terapêuticas evita os problemas relacionados a medicamentos (PRM). O presente estudo tem como objetivo realizar a revisão integrativa da literatura na monitorização farmacoterapêutico da vancomicina em pacientes de unidades de terapia intensiva. Foram encontrados 13 artigos na bases de dados on line, que atenderam aos critérios de inclusão e exclusão da pesquisa. A presença do farmacêutico clínico, nas unidades de tratamento intensivo é de suma importância.
