Vancomycin is an antibiotic of the glycopeptide class and acts on the cell wall of bacteria, disrupting their synthesis, especially in species of methicillin-resistant Staphylococcus aureus (MRSA), coagulase-negative Staphylococcus and Entecococcus faecium. It is a drug that has a nephrotoxic potential, due to its narrow therapeutic window and depending on the patient's hemodynamic stability. The presence of the clinical pharmacist together with the multidisciplinary team is necessary and important. This pharmaceutical professional is responsible for several responsibilities, including pharmacotherapeutic monitoring, which, by adjusting therapeutic doses, avoids drug-related problems (DRP). The present study aims to carry out an integrative review of the literature on the pharmacotherapeutic monitoring of vancomycin in patients in intensive care units. 13 articles were found in the online databases, which met the research inclusion and exclusion criteria. The presence of a clinical pharmacist in intensive care units is extremely important. This professional will perform several duties, including pharmacotherapeutic monitoring for vancomycin, which will guarantee the optimization of therapeutic doses of this antibiotic and avoid the appearance of adverse and/or toxic events, as well as possible failures in drug therapy, which could worsen the patient's clinical status.
