Resumo: Objective: To understand, based on recent literature, the correlation of Polycystic Ovarian Syndrome (PCOS) with Insulin Resistance (IR) identifying risk factors for the onset of Metabolic Syndrome (MS). Method: This is a narrative literature review study with a qualitative approach. The literary survey was conducted in the following databases: Latin American and Caribbean Literature on Health Sciences (LILACS) and Scientific Electronic Library Online (SciELO). Results and Discussion: IR plays a significant role in the etiopathogenesis of PCOS, in addition, excess fat ends up promoting the worsening of IR, generating complications both in metabolic dysfunction and in reproductive and hyperandrogenic characteristics. Conclusion: It is concluded that PCOS is linked to metabolic endocrine changes, associating IR as the main link between PCOS and MS. On the other hand, some factors contribute to the onset of MS, which should be evaluated in regular exams for an early diagnosis.
