Efficacy of drug treatment in mitral valve endocardiosis in elderly dogs: case report

Resumo: Endocardiosis, Myxomatous Mitral Valve Disease (MVD) is the disease that manifests itself most frequently in adult male dogs of small breeds. It is responsible for the development of Congestive Heart Failure (CHF), a chronic degenerative disease that negatively impacts the animal's quality of life and can lead to death. The study aimed to analyze and describe the possible treatment methods available for canine endocardiosis. The specific objectives involved the analysis of studies already carried out to identify the size and breeds most affected by the pathology; compare the evolution of the disease in the most affected breeds; review work already completed in the field of veterinary cardiology. The study reports the clinical case of a Schnauzer dog presenting clinical signs of congestive heart failure, such as cough, dyspnea, exercise intolerance and pulmonary edema. The complementary echocardiography exam showed Laminar Tricuspid Flow, Laminar Aortic Flow m/s and Laminar Pulmonary flow m/s, noting increased systolic diameter of the right ventricle, suggestive of systolic dysfunction; increased left ventricular diastolic diameter, suggestive of left ventricular congestion; left atrium with enlarged dimensions, suggestive of left atrial overload and mitral regurgitation reflux indicating severe mitral endocardiosis. Due to the advanced stage of the disease, intensive treatment was chosen using diuretics, vasodilators, positive inotropes and bronchodilators, to alleviate symptoms and improve the animal's quality of life. The treatment was administered for 48 days, resulting in death on December 14, 2022.

É responsável pelo desenvolvimento da Insuficiência Cardíaca Congestiva (ICC), doença crônica degenerativa que impacta de forma negativa na qualidade de vida do animal, podendo levá-lo a morte. O estudo teve como objetivo analisar e descrever os possíveis métodos de tratamento disponíveis para Endocardiose canina. Os objetivos específicos envolveram a análise de estudos já realizados para identificar o porte e as raças mais acometidas pela patologia; comparar a evolução da doença nas raças mais acometidas; revisar trabalhos já concluídos no âmbito de cardiologia veterinária e relatar o caso clínico de um cão da raça Schnauzer apresentando sinais clínicos de insuficiência cardíaca congestiva, como tosse, dispneia, intolerância ao exercício e edema pulmonar. O exame complementar de ecocardiografia o apresentou Fluxo Tricuspídeo Laminar, Fluxo Aórtico Laminar m/s e fluxo Pulmonar Laminar m/s, constatando-se: aumento do diâmetro sistólico do ventrículo direito, sugestivo de disfunção sistólica; aumento do diâmetro diastólico do ventrículo esquerdo, sugestivo de congestão do ventrículo esquerdo; átrio esquerdo com dimensões aumentadas, sugestivo de sobrecarga atrial esquerda e refluxo de regurgitação mitral indicando endocardiose de mitral severa. Devido ao estágio avançado da doença optou-se pelo tratamento intensivo com uso de diuréticos, vasodilatadores, inotrópicos positivos e broncodilatadores, para aliviar os sintomas e melhorar a qualidade de vida do animal.
