Introduction: The Singular Therapeutic Project is a work carried out by the interdisciplinary health team with a view to monitoring a specific case that involves a subject or a community. Objective: To report the academic assistance experience of Nursing students during the joint elaboration of a unique therapeutic project (STP) at the Guaxuma Family Health Unit during a home visit of a user with an acute chronic disease. Method: This is a qualitative research, of the experience report type, which addresses the experience of nurses, in the mandatory curricular internship in a Family Health Unit, in the construction of a PTS. Results: home visits and weekly meetings with the multidisciplinary health team were carried out to discuss the cases, aiming to align the therapeutic objectives in the short, medium and long term, resulting in the construction of the intervention plan based on the health, social and political needs. singular subject programmatic. Final considerations: The proposal of the PTS allowed the multidisciplinary team to problematize the specific needs of the individual, contemplating comprehensive care in a humanized perspective of care, socialization of knowledge, incorporation of multidimensionality in approaches and the improvement of care practices.
