Terapia fotodinâmica antimicrobiana como coadjuvante ao tratamento da doença periodontal: modelo experimental

Resumo: Antimicrobial photodynamic therapy (aPDT) has been used as an adjuvant treatment for periodontitis. It combines a photosensitizer with a light source to induce reactive oxygen species and kill microbial cells. PpNetNI is a protoporphyrin derivated, and it has a chemical binding site at biofilm and great affinity to microbial cells. The aim of this study was to investigate the effects of aPDT as an adjuvant treatment for periodontitis. Thirty healthy male rats Wistar (Rattus norvegicus) were used in this study (Approved by UNINOVE Ethical committee AN0029/2015). Periodontitis was induced by placing a cotton ligature around the first mandibular molar in a subgengival position. The contralateral mandibular first molar received neither a ligature nor any treatment, and was used as a control. After 7 days, the ligature was removed and all animals received scaling and root planing (SRP) and were divided according to the following treatments: SRP group (received SRP and irrigation with PpNetNI, 10 µM) and PDT group (PpNetNI 10 µM followed by LED irradiation). PDT was performed with a LED (630 nm) with an output power of 400 mW (fluence-rate 200 mW/cm2; fluence 18 J/cm2). Rats were euthanized at 24 h, 48 h and 7 days postoperatively. The area of bone loss in vestibular region of the first molar was evaluated by Optical Coherence Tomography (OCT). Data were analyzed statistically (ANOVA and Tukey tests, p˂0.05). Inflammatory markers IL-1, IL-6 and IL-10 were also evaluated. The animals treated by aPDT showed bone gain of approximately 30% compared to the SRP group following 7 days from the treatment. PDT reduced periodontal tissue destruction and may be useful as an adjuvant to SRP in the treatment of periodontitis.

Ele combina um fotossensibilizador com uma fonte de luz para induzir espécies reativas de oxigênio e destruir células microbianas. A Protoporfirina IX NetNI (PpNetNI) é um derivado de protoporfirina IX que tem sítios de afinidade para ligação às células microbianas. Esta é a primeira utilização deste fotossensibilizador em modelo experimental in vivo. O objetivo deste estudo foi investigar os efeitos da PDT como um tratamento adjuvante para periodontite. Trinta ratos saudáveis Wistar (Rattus norvegicus) foram utilizados neste estudo (Aprovado pelo Comitê de Ética UNINOVE AN0029 / 2015). A periodontite foi induzida pela colocação de uma ligadura de algodão ao redor do primeiro molar mandibular em posição subgengival. O primeiro molar mandibular contralateral não recebeu nem ligadura nem tratamento, sendo utilizado como controle. Após 7 dias, a ligadura foi removida e todos os animais receberam raspagem e alisamento radicular (RAR) e foram aleatoriamente distribuídos de acordo com os seguintes tratamentos: grupo RAR (RAR e irrigação com PpNetNI, 10 μM) e grupo PDT (RAR, PpNetNI 10 μM seguido por LED irradiação). A PDT foi realizada com um LED (630 nm) com uma potência de saída de 400 mW (irradiância 200 mW/cm2, exposição radiante 18 J/cm2). Os ratos foram eutanasiados 24 h, 48 h e 7 dias após o tratamento. A área de perda óssea na região vestibular do primeiro molar foi avaliada por Tomografia de Coerência Óptica (OCT). Os dados foram analisados estatisticamente (ANOVA e testes de Tukey, p˂0,05). Os animais tratados com PDT apresentaram ganho ósseo de aproximadamente 30% em comparação com o grupo RAR após 7 dias do tratamento. E para podermos verificar melhor os resultados, foi feita a análise da expressão de marcadores inflamatórios no tecido periodontal, onde pudemos constatar que 7 dias após o tratamento houve a diminuição da inflamação levando ao reparo do tecido ósseo.
