Resumo: | Background: Serious complications of systemic arterial hypertension (SAH) and preeclampsia (PE) in pregnant women may indicate the interruption of pregnancy and childbirth. Low-level laser therapy (LLLT) was applied in spontaneous hypertensive rats and in SAH patients, results indicated hemodynamics changes. Due to a reduction of the oxidative stress parameters by acting on endothelial dysfunction, that is etiopatogenic base of SAH and PE. Objective: To evaluate immediate response post low-level laser therapy in hypertensive and normotensive pregnant women and possible fetal repercussion. Methodology: Twenty patients of pregnant women (HPW) divided in two groups (normotensive and hypertensive) into 2 phases. All patients rested for 15min between the 2 phases. The hemodynamic parameters with HDI was measured. First phase - placebo (laser off) results of average heart rate, systolic, diastolic and mean blood pressure besides systemic vascular resistance were recorded. Second phase – (laser on) LLLT irradiated through intranasal in the following applied parameters: wavelength (λ) = 808nm, power = 100mW, time = 120s, fluence = 60J/cm2, energy/AP = 6J, total energy = 12J, intensity = 500 mW/cm2, spot size = 0.2 cm2. Cardiotocography monitored the fetuses throughout the entire procedure. Results: The systemic vascular resistance and therefore the systolic, diastolic and mean blood pressure suffered a significant decrease statistically, succeeding photobiomodulation in hypertensive pregnant women. In normotensive pregnant women, such effects were not observed. Considering cardiotocography, no adverse effects of maternal and fetal repercussions were detected. Conclusions: Lasertherapy was able to diminish the systemic vascular resistance and decrease the systemic blood pressure in hypertensive pregnant women. Lasertherapy indicated no side effect on normotensive and hypertensive pregnant women. Thus, the Laser did not produce harm to the fetus, moreover signalized a favorable and feasible fetal repercussion. |

O Laser de Baixa Intensidade (LBI) aplicado em ratos espontaneamente hipertensos e em pacientes hipertensos, resultou em alterações hemodinâmicas. Devido a redução dos parâmetros de estresse oxidativo pela ação na disfunção endotelial, que é a base etiopatogênica da Hipertensão Arterial Sistêmica em grávidas e Pré-eclâmpsia. Objetivo: Avaliar a resposta imediata das condições hemodinâmicas de gestantes normotensas e hipertensas ao laser de baixa intensidade, e possível repercussão fetal. Método: Participaram desse estudo vinte gestantes divididas em dois grupos (normotensas e hipertensas) em duas fases. Todas as pacientes ficaram inicialmente em repouso por 15 minutos, bem como repousaram entre a 1a e 2a fase. Mensuração com HDI dos parâmetros descritos a seguir : Primeira fase – Placebo (laser desligado) e tiveram registrados parâmetros hemodinâmicos com HDI: pressão arterial sistólica, diastólica e média, frequência cardíaca, resistência vascular sistêmica. Segunda fase - Laser de baixa intensidade (LBI), irradiado intranasal com os seguintes parâmetros: comprimento de onda (λ) = 808nm, potência = 100mW, tempo = 120s, Fluência = 60J/cm2, Energia/ponto = 6J, Energia total = 12J, Intensidade = 500mW/cm2, área do feixe de laser = 0,2 cm2. Os fetos foram monitorados por meio da cardiotocografia basal durante todo o processo. Resultados: A resistência vascular sistêmica e, consequentemente, a pressão arterial sistólica e diastólica, pressão arterial média sofreram diminuição estatísticamente significativa, sucedendo a fotobiomodulação em gestantes hipertensas. Em gestantes normotensas, tais resultados não foram observados. Considerando a cardiotocografia não foram observados efeitos adversos maternos e fetais. Conclusão: A terapia com laser de baixa intensidade foi capaz de reduzir, em gestantes hipertensas, a resistência vascular sistêmica e diminuir a pressão arterial sistêmica. A laserterapia não mostrou efeitos colaterais em gestantes normotensas e hipertensas.
