Utilização do cimento de fosfato de cálcio (Norian®) associado a enxertos onlay de calota craniana em região mandibular de ratos: estudo experimental : análise histológica, histométrica e biomecânica

Resumo: This present work has analyzes the integration level of onlay bone grafts of the skull in rats’ mandibular region, combined or not with calcium phosphate cement (Norian ). For this, histology, morphometry, and biomechanical test of traction force have been evaluated. Twenty seven male Wistar rats were subjected to removal of bicortical grafts of the skullcap and fixed in the mandibular angles. The animals were divided into 6 groups: C21d – graft fixation (control – death after 21 days); I21d – graft fixation with Norian powder between the graft and recipient bed (death after 21 days), B21d - graft fixation and placement with Norian , acting as a barrier on the edges of the bone graft (death after 21 days) C55D - graft fixation (control - 55 days after death); I55d - graft fixation with Norian powder between the graft and the recepient bed (death after 55 days); B55d - graft fixation and placement with Norian , acting as a barrier on the edges of the graft (death after 55 days). The hemi-jaws on the left side were taken to the histology and histometric exam and hemi-jaws of the right to a traction force test.There was no infection in none of the cases or postoperative complications. Histological and histometric analysis – Norian ’s remnants were seen in both 21 and in 55 days, showing a gradual replacement by vascularized bone tissue; a higher bone formation was observed in the graft / bone interface receptor (GBIR) on the left side in the groups using the calcium phosphate (B21d, B55d and I55d in relation to C21d) (p = 0. 043); a better result was also observed in regard to the total newly formed bone on the left side interface of these same groups (p = 0. 08); the I55d group presented superior results than the control groups (C21d and C55d) and the group B21d concerning the GBIR bone formation on the right side (p = 0. 045); about the item on the horizontally newly formed bone on the left side, the group C21d had no significant difference in comparison to C55d, however, it presented less bone formation than the other groups that used the calcium phosphate (p = 0. 018). The traction force test - different devices have been developed to achieve an effective fixation of the specimens during the mechanical test, such as the EMIC DL-200 machine and the MTest 3. 0 version program to obtain the strength necessary to remove the grafts measured in Newton (N).The traction test demostrated that the C21d group’s results were significantly lower than other groups (p = 0. 08), furthermore, in two specimens from this group, one graft was not integrated and the other was weakly attached to the bone receptor. Comparing the control groups (21d and 55d), over time, there was a significant increase of the strenght required for the removal of the graft (p = 0. 032). The study showed good results in the use of onlay grafts associated with Norian , presenting higher levels of bone formation, even when dealing with a region of constant muscle stress. Furthermore, there was a better biomechanic integration, especially in the initial and most critical part of healing.

Para isso, foram realizadas avaliações histológicas, histométricas, além de teste biomecânico de força de tração. Vinte e sete ratos Wistar, machos, foram submetidos à remoção de enxertos bicorticais da calota craniana e fixados nos ângulos mandibulares. Os animais foram divididos em 6 grupos: C21 d - fixação do enxerto (controle - morte após 21 dias); 121 d - fixação do enxerto com o pó do Norian® entre o enxerto e o leito receptor (morte após 21 dias); B21 d - fixação do enxerto e colocação de Norian®, atuando como barreira, sobre as bordas do enxerto (morte após 21 dias); C55d - fixação do enxerto (controle - morte após 55 dias); 155d - fixação do enxerto com o pó do Norian entre o enxerto e o leito receptor (morte após 55 dias); B55d - fixação do enxerto e colocação de Norian, atuando como barreira, sobre as bordas do enxerto (morte após 55 dias). As hemimandíbulas do lado esquerdo foram destinadas ao exame histológico e histométrico e as hemi-mandíbulas do lado direito ao teste de força de tração. Em nenhum dos casos ocorreu infecção ou complicações pós-operatórias.Análise histológica e histométrica - resquícios do Norian foram vistos tanto em 21 quanto em 55 dias, demonstrando uma gradual substituição por tecido ósseo vascularizado; foi observada uma maior formação óssea na interface enxerto/osso receptor (TEOR) do lado esquerdo nos grupos que utilizaram o fosfato de cálcio (B21d, B55d e 155d em relação ao C21 d) (p= 0,043); um melhor resultado também foi observado em relação ao osso total neoformado na interface do lado esquerdo, nesses mesmos grupos W-0,08); o grupo 155d apresentou resultados superiores aos grupos controle (C21d e C55d) e ao grupo B21d na formação de osso na IEOR do lado direito (p= 0. 045): no item osso neoformado horizontalmente do lado esquerdo, o grupo C21d não apresentou diferença significativa em relação ao C55d, contudo, demonstrou haver formado menos osso que os demais grupos que utilizaram o fosfato de cálcio (p= 0,018). Teste de força de tração - diferentes dispositivos foram desenvolvidos para conseguir uma fixação eficaz dos espécimes durante o ensaio mecânico, sendo utilizada a máquina EMIC® DL-200 e o programa MTest versão 3. 0 para obtenção das forças de remoção dos enxertos em Newton (N). O teste de tração demonstrou que o grupo C21d apresentou resultados significativamente inferiores aos demais grupos (p= 0,08), além disso, em dois espécimes deste grupo, um enxerto não estava integrado e o outro estava fracamente aderido ao osso receptor. Comparando-se os grupos Controle (21d e 55d), ao longo do tempo, houve um aumento significativo na força necessária para a remoção dos enxertos (p= 0,032). O estudo demostrou bons resultados na utilização dos enxertos onlay associados ao Norian®, apresentando maior nível de formação óssea, mesmo se tratando de uma região de constante estresse muscular.
