Resumo: | Major Depression (MD) poses a great public health problem, where the prevalence in the male adult population varies from 2% to 3%, reaching values 2-3 times as high in females. The association between MD and altered Natural Killer (NK) activity has been repeatedly suggested, but experimental data show contradictory results. NK activity assays were performed in patients with DM, unipolar subtype, and healthy subjects. The NK activity was assayed in a standard 4-hour Cr release assay, using as target the K562 human erythroleukemiaderived cell line. Patients were free from medical disorders clearly associated with immunity disorders such as ongoing infections, cancer, immunodeficiencies and autoimmune disease. The psychiatric diagnoses were made using the DSM-III-R Checklist (kappa = 0,875 for the evaluator). We also measured nutritional parameters (ferritin, folie acid, vitamin B12, transferrin, albumin, hemoglobin, and index of body mass) and integrity of the adrenal axis (cortisol). A rigorous pairing was employed (gender, age, race, socio-economical leveis, tobacco, contraceptive use, menses and physical activity), to exclude externai factors influencing natural immunity. NK activity was significantly lower in patients as compared to healthy controls (p = 0,001). A relationship between NK activity and measure of hypothalamic-pituitary-axis function, such as baseline cortisol secretion, was not found in patients. No significant changes were observed in the index of body mass and others nutritional variables. Patients with familiai history of mental disease presented lower NK activity (p = 0,03). This finding may suggest that the sarne genetic factors are presumably associated to the etiology of depression and NK activity changes |

A associação entre DM e atividade Natural Killer (NK) alterada foi repetidamente sugerida, mas dados experimentais têm revelado resultados contraditórios. Com o objetivo de contribuir para a elucidação do tema, pacientes com DM subtipo unipolar e controles sadios foram avaliados quanto à atividade NK. A mensuração da atividade NK foi realizada através do ensaio padrão que utiliza a liberação de 51Cr e células alvo K562, uma linhagem celular derivada de eritroleucemia. Foram excluídos pacientes que apresentassem doenças que pudessem estar claramente associadas a alterações na imunidade, como infecções ativas, câncer, imunodeficiências ou doenças auto-imunes. O diagnóstico psiquiátrico foi feito com o uso do Inventário da DSM-111-R (avaliador com kappa = 0,875 para DM). Com o objetivo de controlar variáveis que pudessem influir nos achados, realizamos um pareamento do sexo, idade, raça, nível sócio-econômico, tabagismo, uso de contraceptivos orais, história menstrual e atividade fisica Para verificar se as possíveis alterações na atividade NK se deviam a variações ponderais dos pacientes, também foram avaliadas variáveis nutricionais ( ferritina, ácido fálico, vitamina B12, transferrina, albumina, hemoglobina e índice de massa corporal). A influência da ativação do eixo hipotálamohipófise-adrenal (HP A) na atividade NK foi avaliada pela medida do cortisol sérico. Os pacientes foram divididos em leves, moderados e graves de acordo com a severidade do seu quadro clínico. A severidade dos sintomas depressivos foi avaliada pela escala de depressão MADRS. Foi encontrada uma atividade NK significativamente reduzida em pacientes quando comparados com os controles (p = 0,001). No entando, não foi encontrada uma relação entre os níveis séricos e a atividade NK. Nenhuma alteração significativa foi encontrada no índice de massa corporal e nas outras variáveis nutricionais. Também nenhuma variável clínica se relacionou com a atividade NK. Somente os pacientes com história familiar de doença mental apresentaram uma menor atividade NK (p = 0,03).
