Funções executivas na epilepsia de lobo temporal associada à esclerose hipocampal: impacto da ressecção seletiva das estruturas mesiais temporais

Resumo: Purpose: To study the performance of patients with unilateral temporal lobe epilepsy due to hippocampal sclerosis (TLE/HS) in a neuropsychological test battery of executive functions before and after surgical resection of mesial temporal lobe structures to control seizures. Methods: Two studies were carried out. The first compared performance in a neuropsychological test battery of executive and memory functions, as well as in the scores of scales measuring anxiety and depression, of patients with TLE/HS and of a control group of people without neurological or psychiatric disorders matched for age, gender and level of education. In the second study, the performance and scores in the same batteies and scales were compared within the cohort of TLE/HS patients both before and six months after unilateral selective amygdalo-hippocampectomy. Results: Sixty-four % of the 25 patients with TLE/HS completed lass than four categories in the Wisconsin Card Sorting Test (WCST) and 40% had potentially significant abnormalities in the Stroop test. In all tests of executive function, patients had a significantly poorer performance than the controls, except for digit span in the reverse order. A significant improvement after surgery was observed in many subscores of the WCST, including total number of correct matchings, total number of errors, number of perseverative errors and of perseverative responses. Conclusions: Patients with unilateral TLE/HS have significant deficits in tests probing executive function when compared to a control group. Performance in this group significantly improves in many subscores of the WCST following selective resection of the epileptogenic mesial temporal structures.

Metodologia: Foram realizados dois estudos. O primeiro comparou a performance dos pacientes com ELT/EH em testes de função executiva, memória, e também quanto aos escores em uma escala de depressão e ansiedade a um grupo controle de pessoas sem doença neurológica ou psiquiátrica, pareado por sexo, idade e escolaridade. No segundo estudo a performance nos mesmos testes e escalas foram comparadas a uma coorte de pacientes com ELT/EH, antes e após seis meses após amígdalo-hipocampectomia seletiva unilateral. Resultados: Dos 25 pacientes com ELT/EH, 64% completaram menos de quatro categorias no WCST, performance esta altamente sugestiva de disfunção executiva. No Stroop test, 40% dos pacientes tiveram escores abaixo da média no critério cor-palavra com potencial significância do ponto de vista clínico para disfunção executiva. Em todos os testes de funções executivas o grupo de pacientes com ELT/EH teve um desempenho significativamente inferior ao dos controles, exceto em relação a dígitos ordem inversa. Uma melhora significativa da performance no pós-operatório em comparação com aquelas antes da cirurgia foi observada em diversos subescores do WCST, incluindo número total de pareamentos corretos, número total de erros, número de erros perseverativos e número de respostas perseverativas. Conclusão: Os pacientes com ELT/EH apresentaram déficits nos testes de funções executivas.
