Efeito da laserterapia (GaAlAs) no processo de reparo dos tecidos ósseo e mole em ratos submetidos à terapia com Zolendronato

Resumo: | Bisphosphonates are drugs, administered/given either orally or intravenously, that are used to treat bone disorders. They inhibit osteoclast formation by suppressing osteoclast-mediated bone resorption and accumulate persistently in the mineralized bone matrix. Notwithstanding the benefit obtained from their use, treatment with bisphosphonates has been associated with maxillary bone necrosis. In this study, 54 rats were used. The rats were randomized into three different groups: G1 (control group); G2 (group treated with zoledronic acid) and G3 (group treated with zoledronic acid and laser therapy). Each group was divided in 2 subgroups (A and B). In group A, the incisive were extracted. In group B, a bur was used to remove bone from the first molar region.Zoledronic acid treatment was initiated one month before surgery and given in three doses of 0,6 mg/kg. Low-level laser therapy was applied during the procedure and every 48h, at a dose of 1J/cm2, given punctually, using a power of 90 mW and wavelength of λ=830nm. The rats were killed at 44 days after the surgical procedure. Specimens were evaluated macroscopically, and underwent histological and histomorphometric analysis. Statistical analysis included the Chi-Square test, Fisher´s Exact test and Anova test. The results indicated that laser therapy didn´t prevent the osteonecrosis. And that Zolendronic Acid associated with a local pathology has the potencial to increase the occurrence of osteonecrosis. |

Administrados por via oral ou endovenosa, os BPs inibem a atividade osteoclástica suprimindo a remodelação óssea mediada pelos osteoclastos e ficam acumulados de modo persistente na matriz óssea mineralizada. Apesar do benefício de sua utilização, há uma potencial relação entre a terapia com bifosfonatos e a osteonecrose de maxilares. Nesse estudo foram utilizados 54 ratos. Os animais foram selecionados aleatoriamente e divididos, de modo randomizado, em 3 grupos contendo 18 espécimes. Em todos os grupos, o período de observação foi de 03 meses. O grupo G1 foi o controle; o G2 recebeu a medicação sem laserterapia (LLLT) e o G3 foi o grupo teste que recebeu terapia medicamentosa e laserterapia. Cada grupo foi subdividido em 2 subgrupos contendo 9 espécimes. No subgrupo A foi feita a exodontia do incisivo inferior esquerdo; enquanto que no subgrupo B foi realizado um desgaste com broca na região do primeiro molar inferior esquerdo. A terapia medicamentosa foi realizada com o zolendronato e administrada em doses de 0,6 mg/kg, iniciadas um mês antes da cirurgia. Em cada espécime foi aplicada uma dose da solução a cada 28 dias, totalizando 3 aplicações da substância. A exodontia do incisivo inferior ou defeito com broca foi realizado sete dias após a segunda dose do fármaco. A LLLT foi realizada no momento do procedimento e a cada 48h, com dose de 1J/cm2, aplicada de forma pontual com potência de 90 mW e comprimento de onda de 830 nm. As peças foram observadas através das análises macroscópica, histológica e histomorfométrica. A análise estatística foi realizada através dos testes Qui-quadrado, Exato de Fisher e Anova.De acordo com a metodologia utilizada, os 14 autores concluíram que o Zolendronato associado a uma patologia local, potencializa a possibilidade de ocorrência da osteonecrose.
