Adesão medicamentosa em pacientes idosos diabéticos

Resumo: The world and Brazilian population is becoming older, making it important to consider the various aspects that accompany aging, including the physiological changes and diseases usually found in elderly patients. Diabetes mellitus type II, which is very common in this age group, often causes, over the time, an increment in blood glucose, leading to the need of using medicines and contributing to a situation of polypharmacy. Therefore the importance of a proper conduct of the patient with medication for the advancement of the disease to be delayed. One of the problems is the lack of adherence to treatment. The aim of this work is to verify the several variables that may interfere with control of diabetes mellitus in the elderly. For this, there were used two population samples, one from a multidimensional study of the elderly in Porto Alegre (EMIPOA), held at the Institute of Geriatrics and Gerontology (IGG) of the Pontifical Catholic University of Rio Grande do Sul (PUCRS). With this sample there were analyzed socio-demographic and clinical variables and also variables related to diet, compared with the data obtained through the self-reported questionnaire previously answered. The second sample was composed of a group of elderly patients who participated of a clinical trial to verify the effectiveness of the Project "Using of Knowlodge Construction in a Plan of Pharmaceutical Intervention to Promote adhesion to Prescription of hypoglycemic drugs in the elderly." It was also a cross-sectional study where only the data collected before the intervention were used. This project examined the adherence through the questionnaire of self-report and counting the forgotten pill. There were examined socio-demographic, pharmacological, clinical and cognitive variables and memory strategies used by older people to take the medication. The results show that there is influence of age on the compliance, however, on the contrary to what it is imagined, it improves with age. As the number of drugs, which increases with frequency rasing of older members. Another interesting question is the relationship of the strategies of memory with the scores in the questionnaire for membership, and we can also see a decrease in consumption of carbohydrates by members, also suggesting an adherence to the diet. With these results we can develop strategies to improve adherence to medication in this population.

O diabetes mellitus tipo II, muito comum nessa faixa etária, provoca, muitas vezes, ao longo do tempo, um aumento da glicemia, levando à necessidade de uso de medicamentos e contribuindo para uma situação de polifarmácia. Por isso a importância de uma conduta correta do paciente com a terapia medicamentosa, para que o avanço da doença seja retardado. Um dos problemas existentes é a falta de adesão ao tratamento. O objetivo desse trabalho é verificar as mais diversas variáveis que possam interferir no controle do diabetes mellitus em idosos. Para isso, foram utilizadas duas amostras populacionais, uma advinda do Estudo Multidimensional dos Idosos de Porto Alegre (EMIPOA), realizado no Instituto de Geriatria e Gerontologia (IGG) da Pontifícia Universidade Católica do Rio Grande do Sul (PUCRS). Nessa amostra foram analisadas variáveis sócio-demográficas, clínicas e referentes a dieta habitual, comparando com os dados de adesão obtidos através de um questionário autorelatado. A segunda amostra foi composta por um grupo de idosos diabéticos que participaram de um ensaio clínico para verificar a eficácia do Projeto “Emprego da Construção do Conhecimento em um Plano de Intervenção Farmacêutica para Promover a Adesão à Prescrição de Fármacos Hipoglicemiantes em Idosos”. Foi também um estudo transversal onde apenas os dados coletados antes da intervenção foram utilizados. Esse projeto analisou a adesão através de questionário de auto-relato e contagem de comprimidos esquecidos. Foram analisadas variáveis sócio-demográficas, farmacológicas, clínicas, cognitivas e as estratégias de memória utilizadas pelos idosos para tomar a medicação. Os resultados obtidos mostram que existe influência da faixa etária sobre a adesão, porém, ao contrário do que se imagina, a mesma melhora com a idade. Assim como o número de medicamentos, que aumenta com o aumento da frequência de idosos aderentes. Outra questão interessante é a relação das estratégias de memória com a pontuação no questionário de adesão, e também podemos perceber uma diminuição no consumo de carboidratos pelos aderentes, sugerindo uma adesão também à dieta.
