Health councils: “macro” and “micro” visions

Resumo: After introducing main normative aspects of the Brazilian Health Councils, the author discusses them from two contrasting perspectives – one broader and narrow the other. The first one, or “macro”, shows the extraordinary development of the citizens’ participation in public spaces of deliberation over a variety of matters involving large and dense networks of heterogeneous social society organizations together with government officials and other actors. The second one, or “micro”, examines the problems, dilemmas and challenges actually faced by the health councils in attaining its objectives. The exposition concludes pointing out the relevant role played by the health councils as “schools of civic learning”; nevertheless, argues that deficits of social capital and of civil engagement existing in the Brazilian society would negatively affect the legitimacy of those arenas and the efficacy of social control over the health public policy process. Key words: Administrative councils; Health; Social control; Public policies.O artigo apresenta primeiramente aspectos formais dos Conselhos de Saúde, analisando em seguida seu desempenho desde duas perspectivas dissonantes entre si. Sob uma abordagem ampla ou “macro”, mostra-se o extraordinário desenvolvimento da participação cidadã nos espaços públicos de deliberação sobre os mais variados assuntos setoriais, envolvendo redes reais e virtuais de interações entre uma heterogênea e densa gama de organizações da sociedade civil, junto com representantes governamentais e outros. Já desde uma ótica “micro” examinam- se os problemas, dilemas e desafios que apresentam essas arenas no seu funcionamento real, dando ênfase a questões como representação, representatividade e associativismo. Conclui-se que, embora os conselhos constituam espaços reais de educação cívica, o déficit de capital social e de engajamento cívico existente na sociedade em geral incidiria na legitimação dos conselhos enquanto arenas de decisão e na eficácia do controle social sobre a formulação e execução das políticas públicas na saúde.
