The impact of the oncological treatment on the quality of life of elderly with breast cancer assisted by the brazilian Public Health System

Resumo: Aims: to evaluate the quality of life (QV), drug interactions and adherence to treatment in elderly patients with breast cancer who did cancer treatment through the brazilian Public Health System (SUS).Methods: prospective cohort, with six months of follow-up, which included elderly patients with breast cancer treated at the University Hospital of Porto Alegre and assisted by the SUS. Women aged ≥ 60 years were selected, divided into two groups (60-69 years and ≥ 70 years).Results: thirty-eight patients were included in the QV and treatment adherence analysis. Within the molecular classification, the most diagnosed subtypes were Luminal B/Her2- (34,2%), Luminal A (26,3%), Luminal B/HER2+ (21,1%). Physical symptoms most related to the disease, at the time of the diagnosis, were insomnia, musculoskeletal stiffness, concerns regarding other people and the future. In contrast, six months later the less favorable results were fatigue, nausea and vomit, dyspnea, pain, inappetence, constipation, diarrhea, financial issues, systemic treatment adverse events, symptoms in the breasts and arms, and alopecia. Low treatment adherence was identified in 67.6% of patients. Polypharmacy was evidenced in 60.6%, and at least one potential drug interaction was observed in 78.8% of the sample. The average of medicines used was 7.24 (SD= 3.77). Conclusions: this study demonstrated the importance of monitoring the elderly population with breast cancer that uses chemotherapy, in order to understand the implications of senescence, as well as improve the rates of adherence to therapy and the quality of life of this population.

Se seleccionaron mujeres con edad ≥ 60 años, divididas en dos grupos (60-69 años y ≥ 70 años).Resultados: 38 pacientes fueron incluidos en el análisis de CV y adherencia al tratamiento. Dentro de la clasificación molecular, los subtipos más diagnosticados fueron Luminal B/Her2- (34,2%), Luminal A (26,3%), Luminal B/HER2+ (21,1%). Los síntomas físicos más relacionados con la enfermedad, en el momento del diagnóstico, fueron insomnio, rigidez musculoesquelética, preocupación por los demás y por el futuro. En cambio, seis meses después los resultados menos favorables fueron fatiga, náuseas y vómitos, disnea, dolor, inapetencia, estreñimiento, diarrea, problemas económicos, efectos adversos de la terapia sistémica, síntomas en mamas y brazos y alopecia. Se identificaron bajos niveles de adherencia al tratamiento en el 67,6% de los pacientes. Se evidenció polifarmacia en el 60,6% y se observó al menos una posible interacción medicamentosa en el 78,8% de la muestra. El número medio de medicamentos utilizados fue de 7,24 (DE=3,77).Conclusiones: este estudio demostró la importancia del seguimiento de la población anciana con cáncer de mama que utiliza quimioterapia, para comprender las implicaciones de la senescencia, así como mejorar las tasas de adherencia a la terapia y la calidad de vida de esta población.Objetivos: avaliar a qualidade de vida (QV), as interações medicamentosas e a adesão ao tratamento em pacientes idosas com câncer de mama que realizaram tratamento oncológico pelo Sistema Único de Saúde (SUS).Métodos: coorte prospectiva, com seis meses de seguimento, que incluiu pacientes idosas com câncer de mama atendidas em Hospital Universitário de Porto Alegre e atendidas pelo SUS. Foram selecionadas mulheres com idade ≥ 60 anos, divididas em dois grupos (60-69 anos e ≥ 70 anos).Resultados: 38 pacientes foram incluídas nas análises sobre QV e adesão ao tratamento. Dentro da classificação molecular, os subtipos mais diagnosticados foram, Luminal B/Her2- (34,2%), Luminal A (26,3%), Luminal B/HER2+ (21,1%). Os sintomas físicos mais relacionados à doença, no momento do diagnóstico, foram insônia, rigidez musculoesquelética, preocupação com os outros e com o futuro. Em contrapartida, seis meses depois os resultados menos favoráveis foram fadiga, náusea e vômito, dispneia, dor, inapetência, constipação, diarreia, problemas financeiros, efeitos adversos da terapia sistêmica, sintomas nas mamas e braços e alopecia. Baixos níveis de adesão ao tratamento foram identificados em 67,6% das pacientes. A polifarmácia foi evidenciada em 60,6%, e foi observada ao menos uma interação medicamentosa potencial em 78,8% da amostra.
