Factors associated with the presence of urinary incontinence in a group of elderly people examined in a school clinic

Resumo: | Objective: to identify the prevalence of urinary incontinence and associated factors in an elderly population treated in a school clinic. Method: cross-sectional observational study, carried out by searching the medical records of 108 elderly people treated in an interdisciplinary outpatient clinic of a medical school located in the city of São Paulo. People over 60 years of age were included, with care provided from the second half of 2015 to June 2019. The following variables were collected: gender, age, education, marital status, diseases being treated for, existence of polypharmacy, self-perception of health, performance in activities of daily living and frequency and recurrency of falls. For women, the presence or absence of estrogen replacement was verified and for men, the presence of prostatic hyperplasia. Regarding urinary incontinence, its prevalence, type, whether limiting or not, and the existence of nocturia were observed. All variables were analyzed together and according to gender and age. To identify the factors that influence the presence of urinary incontinence, Logistic Regression was performed. Results: most people were between 60 and 69 years old, had incomplete primary education and had a good perception of their own health, being independent in basic activities. Forty-one people had incontinence, which was often associated with the presence of diabetes, polypharmacy and, in women, the absence of estrogen replacement. Conclusion: the study shows the importance of investigating the syndrome in the geriatric population, since it’s still linked to taboos and underdiagnosed, despite its verifiable impact on the quality of life of the elderly. |

Método: estudio observacional transversal, realizado mediante búsqueda en las historias clínicas de 108 ancianos atendidos en un ambulatorio interdisciplinario de una clínica escolar ubicada en la ciudad de São Paulo. Se incluyeron personas mayores de 60 años, con atención brindada desde el segundo semestre de 2015 hasta junio de 2019. Se recogieron las siguientes variables: sexo, edad, escolaridad, estado civil, enfermedades en tratamiento, existencia de polifarmacia, autopercepción de salud, desempeño en actividades de la vida diaria y referencia a caídas. En las mujeres se verificó la presencia o ausencia de reposición de estrógenos y en los hombres la presencia de hiperplasia prostática. Respecto a la incontinencia urinaria se observó su prevalencia, tipo, limitante o no, y existencia de nicturia. Todas las variables se analizaron de forma conjunta y según sexo y edad. Para identificar los factores que influyen en la presencia de incontinencia urinaria se realizó Regresión Logística. Resultados: la mayoría de las personas tenían entre 60 y 69 años, tenían educación primaria incompleta y tenían buena percepción de salud, siendo independientes en las actividades básicas. Cuarenta y una personas padecían incontinencia, que se asoció con la presencia de diabetes, polifarmacia y, en las mujeres, ausencia de reposición de estrógenos. Conclusión: el estudio muestra la importancia de investigar el síndrome en la población geriátrica, que aún está vinculado a tabúes y subdiagnosticado, a pesar de su impacto en la calidad de vida de los ancianos. Incontinência urinária em um grupo de pessoas idosas Objetivo: identificar a prevalência de incontinência urinária e fatores associados em uma população idosa atendida em um ambulatório de uma clínica-escola. Método: estudo observacional transversal, realizado mediante busca em prontuários de 108 pessoas idosas atendidas em um ambulatório interdisciplinar de uma clínica-escola localizada no município de São Paulo. Foram incluídas pessoas acima de 60 anos, com atendimento realizado desde o segundo semestre de 2015 até junho de 2019. Coletaram-se as variáveis: sexo, idade, escolaridade, estado civil, doenças em tratamento, existência de polifarmácia, autopercepção de saúde, desempenho nas atividades de vida diária e referência a quedas. Para as mulheres, verificou-se a presença ou não de terapia hormonal estrogênica e, para os homens, a presença de hiperplasia prostática. Quanto à incontinência urinaria, observou-se sua prevalência, tipo, se limitante ou não, e existência de noctúria. Todas as variáveis foram analisadas em conjunto e conforme sexo e idade. Para identificar os fatores que influenciam a presença de incontinência urinária foi feita Regressão Logística. Resultados: a maioria das pessoas tinha entre 60 e 69 anos, ensino fundamental incompleto e boa percepção de saúde, sendo independentes para as atividades básicas. Quarenta e uma pessoas apresentaram incontinência, sendo que esta mostrou-se associada à presença de diabetes, de polifarmácia e, nas mulheres, a ausência de terapia hormonal estrogênica.
