Prevalence of factors related to frailty and physical mobility in older people with heart disease: a cross-sectional study in an outpatient setting

Resumo: | Introduction: frailty is a multifactorial syndrome of biological, psychological, and social conditions that p redisposes the older people to a decline in functionality and greater susceptibility to vulnerability. Such a condition can be exacerbated in those presenting comorbidities, with cardiovascular diseases being particularly notable. Objective: To ascertain the prevalence of factors related to frailty and physical mobility, as well as the correlation between these indicators in elderly individuals in an outpatient setting. Case material and methods: a cross-sectional study with an exploratory, descriptive, and quantitative approach evaluated a group of older individuals with chronic cardiovascular diseases of both sexes, aged ≥60 years, undergoing care at a cardiology outpatient clinic. Participants were categorized according to Fried’s physica’ frailty classification, and their physical mobility was assessed using the Timed Up and Go (TUG) test, muscular strength through handgrip strength (HGS), muscular quantity of calf muscles through calf circumference (CC), and multidimensional frailty with the Índice de Vulnerabilidade Clínico-Funcional (IVCF-20). Prevalence was investigated through relative andabsolute frequency, and for correlation analyses, the Spearman test was employed. Results: the median age of the sample was 73 years; 41 (61.2%) were frail, 17 (25.4%) pre-frail, and 9 (13.4%) non-frails. Concerning physical mobility, the frail group exhibited muscular weakness with HGS of 15.3 kgf (1.6 – 33.3) and a longer required time in the TUG test of 16.3s (8.8 – 50.0). There is evidence of a correlation between IVCF-20 and TUG (p= <0.001; r=0.580), as well as a negative correlation between HGS and TUG (p= <0.001; r= -0.434) and IVCF-20 and HGS (p= <0.001; r= -0.378). Conclusion: there is a prevalence of physical frailty in elderly individuals with cardiovascular diseases undergoing outpatient care. Mobility measures are correlated with each other. |

Objetivo: conocer la prevalencia de factores relacionados con la fragilidad y la movilidad física, así como la correlación entre estos indicadores en personas mayores en un entorno ambulatorio. Casuística y método: se llevó a cabo un estudio transversal con un enfoque exploratorio, descriptivo y cuantitativo que evaluó a un grupo de personas mayores com cardiopatías crónicas, de ambos sexos, con una edad ≥60 años que estaban siendo atendidas en una clínica de cardiología. Los participantes fueron classificados según la clasificación de fragilidad física de Fried y se evaluó la movilidad física mediante el Timed Up and Go (TUG), la fuerza muscular a través de la fuerza de prensión manual (FPM), la cantidad muscular de los músculos de la pantorrilla a través de la circunferencia de la pantorrilla (CP) y la fragilidad multidimensional con el Índice de Vulnerabilidad Clínico-Funcional (IVCF-20). La prevalencia se investigó mediante frecuencia relativa y absoluta, y para los análisis de correlación se utilizó la prueba de Spearman. Resultados: la mediana de edad de la muestra fue de 73 años; 41 (61,2%) eran frágiles, 17 (25,4%) prefrágiles y 9 (13,4%) no frágiles. En cuanto a la movilidad física, el grupo de personas frágiles presentó una debilidad muscular FPM de 15,3 kgf (1,6 – 33,3) y un mayor tempo necesario en el TUG de 16,3s (8,8 – 50,0). Existen evidencias de correlación entre IVCF-20 x TUG (p=<0,001; r=0,580), así como una correlación negativa entre FPM x TUG (p= <0,001; r= -0,434) y IVCF-20 x FPM (p= <0,001; r= -0,378). Conclusión: existe una prevalencia de fragilidad física en personas mayores con cardiopatías que están siendo atendidas de forma ambulatoria. Objetivo: conhecer a prevalência de fatores relacionados à fragilidade e à mobilidade física, bem como a correlação entre esses indicadores em pessoas idosas em segmento ambulatorial. Casuística e método: estudo transversal com uma abordagem exploratória, descritiva e quantitativa que avaliou um grupo de pessoas idosas com cardiopatias crônicas, de ambos os sexos, com idade ≥60 anos em acompanhamento em um ambulatório de cardiologia. Os participantes foram separados de acordo com a classificação de fragilidade física de Fried e avaliadas a mobilidade física pelo Timed Up and Go (TUG), a força muscular por meio da força de preensão manual (FPM), quantidade muscular dos músculos da panturrilha através da circunferência de panturrilha (CP) e a fragilidade multidimensional com o Índice de Vulnerabilidade Clínico-Funcional (IVCF-20). A prevalência foi investigada por meio de frequência relativa e absoluta e, para as análises de correlação, o teste de Spearman foi utilizado. Resultados: a mediana da idade da amostra foi de 73 anos; 41 (61,2%) eram frágeis, 17 (25,4%) pré-frágeis; e 9 (13,4%) não frágeis. Com relação à mobilidade física, o grupo de frágeis apresento u fraqueza muscular FPM 15,3 kgf (1,6 – 33,3) e maior tempo necessário no TUG 16,3s (8,8 – 50,0). Há evidências de que exista correlação entre o IVCF-20 x TUG (p= <0,001; r=0,580), além de correlação negativa entre FPM x TUG (p= <0,001; r= -0,434) e IVCF-20 x FPM (p= <0,001; r= -0,378). Conclusão: há prevalência de fragilidade física em pessoas idosas cardiopatas em acompanhamento ambulatorial.
