Resumo: | Disorders of acid-base equilibrium are common in critically ill patients, and are associated with greater morbidity and mortality. Methods to understand these disorders and to identify interventions to avoid them or to treat them are therefore extremelly important. The traditional approach is often inadequate to explain the acid-base disorders and to guide treatment. In the present review, the authors present the Stewart model, which modified the understanding of acid-base disorders in critically ill patients and has brought important clinical contribution. Métodos que permitam entender a natureza destes distúrbios e conseqüentemente identificar atitudes que possam evitá-los e/ou corrigi-los são portanto extremamente importantes. A avaliação tradicional tem se mostrada incapaz de explicar estes distúrbios e auxiliar na escolha das intervenções. Os autores revisam neste artigo o modelo de Stewart, que transformou o entendimento dos distúrbios ácido-básicos em pacientes críticos e trouxe importantes implicações clínicas.
