Importance of vitamin B12 screening in clinical evaluation of elderly patient [Abstract in English]

Resumo: Objective: The aim of this work was to present a literature review about B12 vitamin focusing its importance for elderly patients, deficiency manifestations and therapeutics. Data source: Scientific articles and books published from 1977 to 2004 were reviewed. Data synthesis: B12 vitamin deficiency occurs frequently among elderly people with prevalence higher then 20%. Nevertheless it is often unrecognized because clinical manifestations are subtle and delayed diagnosis results in serious hematological and neurological consequences. Conclusions: Screening of vitamin B12 deficiency is an important procedure in the clinical evaluation of elderly patients, as the early diagnosis can improve the patient’s quality of life by avoiding neurological and hematological disturbances. Objetivo: O presente estudo teve por objetivo apresentar uma revisão da literatura sobre a importância da vitamina B12 na avaliação clínica do paciente idoso, abordando suas funções no organismo, os problemas causados pela deficiência e o tratamento indicado. Fonte de dados: As informações foram obtidas de livros e artigos científicos publicados no período compreendido entre 1977 e 2004. Síntese de dados: A deficiência de vitamina B12 é freqüente entre as pessoas idosas, atingindo prevalência superior a 20%. Entretanto, suas manifestações clínicas iniciais são sutis, e o retardo do diagnóstico pode ter sérias conseqüências neurológicas e hematológicas. Conclusão: A investigação da deficiência de vitamina B12 é um procedimento importante na avaliação clínica do paciente idoso, já que o diagnóstico precoce pode evitar distúrbios neurológicos e hematológicos, bem como proporcionar melhor qualidade de vida ao paciente.
