Peritoneal dialysis: patient education based on the self-care model [Abstract in English]

Resumo: Objectives: Peritoneal dialysis is a treatment option for end-stage renal disease. Such modality of renal replacement therapy is usually performed as a home based treatment. Patient and family commitment is essential for success. Teaching and training patients to proficiently selfcare are a nurses’ role. The nursing model that is best adapted for educating uraemic patients is based on the Orem’s self-care theory. The present paper describes the model used at the Dialysis Unit at Hospital São Lucas of PUCRS (HSL/PUCRS) and reviews the methodology for educating chronic renal failure patients. Data source: Peritoneal dialysis educational program experience at HSL/PUCRS. Data summary: Orem’s self-care model is employed at HSL/PUCRS since 1986, aiming for patient safety and independence. The dialysis nurse evaluates patient motivation, manual ability and knowledge. An individualized training, developing theoretical aspects and dialysis practice follows. Conclusion: the self-care model can be safely employed to educate peritoneal dialysis patients, improving quality of life and leading to patient independence. Objetivos: A diálise peritoneal é um dos tratamentos para o paciente com insuficiência renal crônica, em que a participação do paciente e família é fundamental para o seu sucesso. Habilitar o paciente para o autocuidado é função da equipe de enfermagem, que realiza a instrução e treinamento. O modelo de enfermagem que melhor se adapta para a educação destes pacientes é baseado na teoria do auto cuidado de Orem. Esta revisão tem objetivo de descrever o modelo de educação empregado na Unidade de Diálise do Hospital São Lucas da Pontifícia Universidade Católica do Rio Grande do Sul (HSL-PUCRS) e revisar a metodologia de ensino para o paciente urêmico crônico. Fonte de dados: Experiência do programa educacional de diálise peritoneal do HSL-PUCRS. Síntese de dados: O modelo do autocuidado de Orem é utilizado desde 1986, visando a máxima independência e segurança do paciente. A enfermeira de diálise peritoneal avalia a motivação, habilidades manual e cognição. A seguir, realiza treinamento teórico/ prático, que é individualizado para as características de cada paciente. Conclusões: a técnica de autocuidado pode ser empregada para educação dos pacientes renais crônicos de maneira eficaz e segura, proporcionando melhor qualidade de vida e independência.
