Overweight and the need for drug treatment in women with gestational diabetes

Resumo: Objective: To investigate the association between maternal AIMS: To investigate the association between overweight and need for drug treatment in women with gestational diabetes. METHODS: A retrospective cross-sectional study was conducted in the Hospital Dona Helena, in Joinville, Santa Catarina state, using data collected from medical records of pregnant women with gestational diabetes mellitus. The included period was July 2001 to July 2011 and the variables investigated were: height, pre-pregnancy weight and body mass index, weight at the first visit, body weight, total weight gain and type of treatment prescribed (diet therapy or medication). RESULTS: The study included 320 pregnant women, of whom 134 (41.8%) were overweight before pregnancy. One hundred and seventy-eight patients (55.6%) required medical treatment (oral hypoglycemic agents or insulin) and 142 (44.4%) continued treatment with diet only. The average initial body mass index of patients in need of drug treatment was higher compared to those that were maintained on the diet alone (26.1 vs. 23.1, p less than 0.01) and average initial weight (75.5 vs. 69.3, p

MÉTODOS: Foi realizado um estudo transversal retrospectivo no Hospital Dona Helena em Joinville, estado de Santa Catarina, a partir de dados coletados dos prontuários de gestantes com diabetes mellitus gestacional. O período incluído foi julho de 2001 a julho de 2011 e as variáveis investigadas foram: estatura, peso e índice de massa corporal pré-gestacionais, peso na primeira consulta, peso final, ganho ponderal total e tipo de tratamento prescrito (dietoterápico ou medicamentoso). RESULTADOS: O estudo incluiu 320 gestantes, das quais 134 (41,8%) apresentaram excesso de peso pré-gestacional. Cento e setenta e oito gestantes (55,6%) necessitaram de tratamento medicamentoso (hipoglicemiante oral ou insulina) e 142 (44,4%) mantiveram o tratamento apenas com a dieta. A média de índice de massa corporal inicial das pacientes com necessidade de tratamento medicamentoso foi maior quando comparada com aquelas que permaneceram apenas com a dieta (26,1 vs. 23,1, p menor do que 0,01), assim como as médias de peso inicial (75,5 vs. 69,3, p menor do que 0,01) e de peso final (77,4 vs. 70,8, p menor do que 0,01). A necessidade de tratamento medicamentoso foi maior entre as gestantes classificadas com sobrepeso e obesidade (odds ratio=9,6; intervalo de confiança 95% 3,3-27,8) e naquelas com o ganho de peso gestacional total acima das recomendações (p=0,02).
