Autorreferência de problemas auditivos e vestibulares em mulheres com tireoidite de Hashimoto

Resumo: Introduction: Hashimoto's thyroiditis is an autoimmune thyroid disease in which the immune system attacks the gland itself with high concentrations of antithyroid antibodies. It is one of the most common endocrinological diseases and the most common cause of hypothyroidism. It usually appears more in women than in men, between 30 and 50 years of age, although it can occur in adolescents and young women. Research shows that there is a strong association between the concentration of antithyroid antibodies and the hearing organ, suggesting that their circulation in the blood is a risk factor for hearing loss, dizziness and tinnitus. In this context, no research focusing specifically on self-reported hearing and vestibular problems in the population with Hashimoto's thyroiditis was found. Objective: To investigate complaints of hearing and vestibular problems in women with Hashimoto's thyroiditis. Method: This is an observational, cross-sectional survey conducted with 140 women aged 18 to 59 years from endocrinology practices and a public hospital in the city of Porto Alegre/RS. Two study groups were constituted: one with Hashimoto's thyroiditis (GH) and one control without Hashimoto's thyroiditis (CG). The data collection instrument used was a health survey with personal data, history of diseases and drug treatments, self-referral questions and visual analog scales of hearing and balance. Of the 140 women in the sample, 55 also performed conventional audiometric assessment. Results: The proportion of women who reported hearing loss is higher in the Hashimoto group (63.8%) than in the Control group (17.1%). The proportion of women who reported feeling dizzy and tinnitus is higher in the group. Hashimoto (34.3% and 55.7%, respectively) than in the Control group (5.7% and 20.0%, respectively) and the chance of a Hashimoto individual not hearing as before is 9.78 times the chance that an individual in the Control group will not hear as before. In the audiometric analysis, the Hashimoto group had more hearing loss than the control group (44.4% and 10.7%, p = 0.005). Conclusion: This paper shows that Hashimoto's thyroiditis is a risk factor for complaints of hearing and vestibular problems

É uma das doenças endocrinológicas mais frequentes e a causa mais comum de hipotireoidismo. Costuma aparecer mais em mulheres do que em homens, entre 30 e 50 anos de idade, embora possa ocorrer em adolescentes e mulheres jovens. Pesquisas demonstram que existe uma forte associação entre a concentração dos anticorpos antitireoidianos e o órgão da audição, sugerindo que a circulação deles no sangue é um fator de risco para a perda de audição, tontura e zumbido. Neste contexto, não foram encontradas pesquisas com foco específico em autorreferência de problemas auditivos e vestibulares na população com tireoidite de Hashimoto. Objetivo: Investigar queixas de problemas auditivos e vestibulares em mulheres com tireoidite de Hashimoto. Método: Trata-se de um estudo observacional, transversal de inquérito, realizado com 140 mulheres de 18 a 59 anos provenientes de consultórios de endocrinologia e de um hospital público, no município de Porto Alegre/RS. Foram formados dois grupos de estudo: um grupo com tireoidite de Hashimoto (GH) e um grupo controle sem tireoidite de Hashimoto (GC). O instrumento de coleta de dados utilizado foi um inquérito de saúde com dados pessoais, histórico de doenças e tratamentos medicamentosos, questões de autorreferência e escalas visuais analógicas de audição e equilíbrio. Resultados: A proporção de mulheres que relataram ter perda auditiva é maior no grupo Hashimoto (63,8%) do que no grupo Controle (17,1%), a proporção de mulheres que relataram sentir tontura atualmente e sentir zumbido é maior no grupo Hashimoto (34,3% e 55,7%, respectivamente) do que no grupo Controle (5,7% e 20,0%, respectivamente) e a chance de um indivíduo do grupo Hashimoto não ouvir como antes é 9,78 vezes a chance de um indivíduo do grupo Controle não ouvir como antes.
