EMDR como possibilidade de psicoterapia para o luto: um estudo de caso instrumental coletivo

Resumo: This qualitative exploratory research is configured as a collective instrumental case study. A consistent bibliographic review was conducted on the use of EMDR with bereaveds. Given the need to deepen the knowledge about the subject, the research was designed to collaborate in the knowledge of the use of EMDR in bereavement. Six participants aged 33 to 52 volunteered to participate from their own perception that they needed help in the bereavement after the death of a loved one for at least two years. The initial evaluation was performed to verify the pertinence of their participation, observing the clinical care of the EMDR, as well as the objectives of the psychotherapeutic process and the survey of the targets to be worked through the treatment plan. As instruments were used the Initial Clinical Assessment (AI), Hogan Grief Reaction Checklist (HGRC), Dissociation Experience Scale – II (DES- II), Beck Depression Scale (BDI), Treatment Plan (PTr), Basic Protocol EMDR, Clinical Records, Final Clinical Assessment (FA), Field Diary and Personal Notes. After the initial evaluation, complaints were raised with a grief focus for the establishment of an individual PTr. Each participant performed between 8 to 10 sessions, and after the end of the intervention had their final evaluation. In the first analysis, the cases were studied individually. In the second analysis, a multiple case study was performed. The comparisons of the assessments provided by the application of HGRC, DES-II and BDI were in agreement with the qualitative results observed in all the participants, in the reduction of the disadaptive memory disturbance, namely: reduction or remission of several physical symptoms; relief when perceiving the accommodation of the worked memory; emergence of positive memories; organization of memories building meaning and/or personal growth; balance between loss orientation and restoration orientation; the emergence of an internal representation indicating the development of a continuous link. In conclusion, EMDR proved to be adequate as a psychotherapeutic intervention for working with variations of complicated grief that composed this collective instrumental case study. Inicialmente foi realizado um consistente levantamento bibliográfico sobre a utilização do EMDR com enlutados. Diante da necessidade de aprofundar o conhecimento a respeito do tema, a pesquisa foi idealizada para colaborar para o conhecimento do uso do EMDR em situações de luto. Seis participantes com idades entre 33 a 52 anos se voluntariaram a participar a partir de sua própria percepção de que precisavam de ajuda em relação ao processo de luto por perda de ente querido há, no mínimo, dois anos. A avaliação inicial foi realizada para verificar a pertinência de sua participação, em observação aos cuidados clínicos do EMDR, bem como para estabelecer os objetivos do processo psicoterápico e o levantamento dos alvos a serem trabalhados por meio do plano de tratamento. Como instrumentos foram utilizados Avaliação Clínica Inicial (AI), Hogan Grief Reaction Checklist (HGRC), Escala de Experiência de Dissociação – II (DES-II), Escala Beck de Depressão (BDI), Plano de Tratamento (PTr), Protocolo Básico do EMDR, Prontuários, Avaliação Clínica Final (AF), Diário de Campo e Caderno de Anotações Pessoais. Após a realização da avaliação inicial, foram levantadas as queixas com foco no luto e para o estabelecimento de um PTr individual, cada participante realizou entre 8 a 10 sessões e, após o encerramento da intervenção, passou pela avaliação final. No primeiro movimento de análise, foram estudados os casos de forma individual. No segundo movimento de análise, foi realizado o estudo do conjunto múltiplo de casos. As comparações das avaliações proporcionadas pela aplicação do HGRC, DES-II e BDI apresentaram-se em concordância com os resultados qualitativos observados em todos os participantes e na redução da perturbação das memórias desadaptativas, a saber: redução ou remissão de diversos sintomas físicos; alívio ao perceber a acomodação da memória trabalhada; surgimento de memórias positivas; organização das memórias; construção de significado e/ou crescimento pessoal; equilíbrio entre a orientação para a perda e a orientação para a restauração; surgimento de uma representação interna indicando o desenvolvimento de um vínculo contínuo. Concluindo, o EMDR mostrou-se adequado como intervenção psicoterápica para o trabalho com variações do luto complicado que compuseram este estudo de caso instrumental coletivo.
