Complicações no processo de alimentação de pacientes bariátricos: aspectos orgânicos, psíquicos e nutricionais

Resumo: Introduction: Bariatric surgery (CB) constitutes the most important advancement in the treatment of severe obesity in recent years. However, the effectiveness of the procedure depends on the achievement of goals in the post-operative period with adaptation of organics, psychics and social capabilities that include rehabilitation feed. Yet these patients are subject to various food complications subsequent to surgery, problems classified as organics, psychics or both. Therefore, it becomes very importante an in-depth study of these problems of organic and psychic nature and its impact on nutritional status, intervening with preventive and corrective measures by specialized professionals. Objective: To investigate the possible complications in the process of feeding of patients undergoing bariatric surgery with the technique of RYGBP, in a biopsychic approach. Method: Retrospective study of clinical quantitative nature, performed at the Ambulatory Surgery Bariatric of a public hospital in the countryside of Sao Paulo, according to established ethical standards for research involving human beings. Casuistry: 47 patients selected from a group of 183 medical records of patients operated on CB, who had complications in the process of feeding after CB with organics, psychics and both symptoms. Of the 47 patients, the age group was of 27 to 63 years old, five male and 42 female. Procedure: A survey and categorization of organics and psychics symptoms, from the analysis of medical records in period of February 2004 to December 2010, followed by statistical analysis of the collected material. Results and Discussion: 31 patients (65.95%) had psychics symptoms: emotional dependence, anxiety and anguish, distorted body image, obsessive compulsive disorders and depression according to psychological and psychiatric evaluation, 12 patients (25.53%) had organics symptoms: displacement and removal of containment ring, incision hernia, gastro esophageal reflux, cholecystitis and gastric fistulas according to medical evaluation, and four patients (8.51%) had both symptoms. The results indicated that there was a significant predominance for psychics problems in relation to organics and both (74.5 vs. 34.0 vs. 8.5). Conclusion: The results insinuate the need to invest in biopsychic approach of these patients with adjustments in traditional nutritional case histories (daily and recall) covering psychics investigations related to nutritional imbalances after surgery

Contudo, a eficácia do procedimento, depende do cumprimento de metas no pós-operatório com adaptações das capacidades orgânicas, psíquicas e sociais, que incluem a readaptação alimentar. Porém estes pacientes estão sujeitos a diversas complicações alimentares subseqüentes à cirurgia, classificados em problemas orgânicos, psíquicos ou ambos. Portanto, se torna de vital importância o estudo aprofundado desses problemas de natureza orgânicas e psíquicas e o seu impacto no estado nutricional, intervindo com medidas preventivas e corretivas por profissionais especializados. Objetivo: Investigar as possíveis complicações no processo de alimentação dos pacientes submetidos à cirurgia bariátrica com a técnica de RYGBP, numa abordagem biopsíquica. Método: estudo retrospectivo de natureza clínico quantitativa, realizado no Ambulatório de Cirurgia Bariátrica de um hospital público do interior de São Paulo, de acordo com as normas éticas estabelecidas para pesquisas com seres humanos. Casuística: 47 pacientes selecionados de um grupo de 183 prontuários de pacientes operados de CB, que apresentaram complicações no processo de alimentação pós CB, com sintomas orgânicos, psíquicos e ambos. Dentre os 47 pacientes, a faixa etária foi de 27 - 63 anos, sendo cinco do sexo masculino e 42 do sexo feminino. Procedimento: Levantamento e categorização dos sintomas orgânicos e psíquicos, a partir da análise de prontuários no período de fevereiro de 2004 a dezembro de 2010, seguido de análise estatística do material coletado. Resultados e Discussão: 31 pacientes (65,95%) apresentaram sintomas psíquicos: dependência afetiva, ansiedade e angústia, imagem corporal distorcida, transtornos compulsivos obsessivos e depressão segundo avaliação psicológica e psiquiátrica, 12 pacientes (25,53%) apresentaram sintomas orgânicos: deslocamento e retirada de anel de contenção, hérnia incisional, refluxo gastro esofágico, colecistite e fistulas gástricas segundo avaliação médica, e quatro pacientes (8,51%) apresentaram os dois sintomas. Os resultados indicaram que houve uma predominância significativa para os problemas psíquicos em relação aos orgânicos e ambos (74,5 vs 34,0 vs 8,5).
